翼状胬肉泪膜稳定性变化的研究

摘　　要：目的:探讨翼状胬肉泪膜稳定性的变化。方法:选取单眼原发性进行期翼状胬肉患者30例;以翼状胬肉眼为观察组,另一眼为对照组。两组分别检查并记录泪液分泌试验、泪液羊齿状试验、泪膜破裂时间、角膜荧光素染色及结膜印迹细胞学检查。将结果应用SPSS12.0统计软件处理相关数据。结果:观察组Sch irm erⅠ试验与对照组间差异无统计学意义(P>0.05)。泪液羊齿状试验观察组羊齿状结晶级别明显高于对照组,两组间差异有统计学意义(P<0.05)。角膜荧光素染色阳性率观察组明显高于对照组,两组间差异有统计学意义(P<0.05)。观察组BUT平均为8.7 s,对照组平均为13.9 s,两组间差异有统计学意义。观察组结膜鳞状上皮化生级别较对照组的级别高(P<0.05)。结膜鳞状化生级别与BUT存在负相关(P<0.05)。结论:翼状胬肉患者泪膜稳定性下降。Objective： To explore the change of tear film stability in pterygium.Methods： 30 cases of progressing primary pterygium in one eye were included.The pterygium eyes were considered as case group and the other eyes as normal control group.All involved eyes were undergone the examination of Shirmer I test,TFT test,BUT test,corneal fluorescein staining and impression cytology.The grades of squamous metaplasia of conJunectivae were classified according to the chart of Nelson＇s classification.The data were processed through the computer with statistical software of SPSS12.0（α=0.05）.Results： There was no statistic difference between groups in Shirmer I test（P0.05）;TFT test showed the case group had higher grade than the control group（P0.05）.The difference between groups in corneal fluorescein staining was significant（P0.05）.BUT tests were shorter in case group than those in control group（P0.05）.Impression cytology showed the case group had higher grade of squamous metaplasia than control group（P0.05）.Shortage of BUT of case group was negatively correlated with the grade of squamous metaplasia.Conclusion： Tear functions are abnormal in pterygium eyes,and the stability of tear film is lower.
