种痘样水疱病样皮肤NK细胞淋巴瘤1例

摘　　要：目的:报告1例"面、躯干、四肢反复起丘疱疹伴发热6月"的8岁患儿的组织病理及免疫组织化学特征,探讨本病与EB病毒感染的关系,以求做出明确诊断。方法:进行组织病理学、免疫组化及EB病毒原位杂交等检查。结果:组织病理学显示真皮层皮肤附件及小血管周围异型淋巴细胞浸润。免疫组化显示真皮淋巴细胞LCA(+),CD3(+),CD45RO(+),CD56(+),TIA-1(+),Ki67(30%+)。皮肤组织及外周血中检测到EB病毒基因。结论:种痘样水疱病淋巴瘤组织病理学特征通常是有小到中等大小的淋巴细胞浸润,没有明显的异型性和多形性。进一步检查包括免疫组化和EB病毒编码的RNA原位杂交检查对于早期诊断至关重要。Objective：To detect a case of an 8-year-old boy, who had fever, a relapsing papulovesicular eruption on his face, trunk and extremities for six months for its histopathological and immunohistochemical features, in order to clarify its diagnosis and explore its relationship with Epstein-Barr virus. Methods： Histopathology, immunohistochemisty and in situ hybridization for EBV were performed. Results：Histological examination demonstrated atypical lymphocytes infiltration in the dermis around the skin accessories and small blood vessels. Immunohistochemical examination showed that lymphocytes were highlighted with markers for LCA （＋）, CD3 （＋）, CIMSRO（＋）, CD56（＋）, TIA-1 （＋）, Ki67 （30%＋）. The EBV genomes were found in the skin specimen and peripheral blood. In the peripheral blood, the percentage of natural killer cells in- creased to 60.3%. Conclusion：Histologically, the lymphocytes are usually small to medium sized without obvious heteromorphism and pleomorphism in HV-like lymphoma. Immunohistochemistry examination and in situ hybridization for EBV encoded RNA are vital to early diagnosis.
