多西他赛联合奥沙利铂治疗晚期头颈部恶性肿瘤

摘　　要：目的:观察多西他赛联合奥沙利铂治疗晚期头颈部肿瘤的疗效及不良反应。方法:52例晚期头颈部肿瘤患者按住院号随机分为A组26例和B组26例。A组多西他赛75mg/m^2,静脉滴注,第1天;奥沙利铂130mg/m^2,静脉滴注,第1天;3周为1周期,连用2周期～4周期。B组顺铂25mg/m^2,静脉滴注,第1天～3天;5-氟脲嘧啶3.5g/m^2,用输液泵连续滴注5天;3周为1周期,连用2周期～4周期。两组均在化疗结束2周后评价疗效。结果:52例均可评价疗效,每例患者均完成2周期以上化疗。有效率A组为53.8%(14/26),B组为42.3%(11/26),两组差异无统计学意义(P>0.05)。1年生存率A组为42.3%,B组为30.8%,两组差异无统计学意义(P>0.05)。生活质量提高A组50.0%(13/26),B组26.9%(7/26),两组差异有统计学意义(P<0.05)。骨髓抑制发生率两组差异无统计学意义(P>0.05)。胃肠道反应A组较B组低(P<0.05)。结论:多西他赛联合奥沙利铂方案治疗晚期头颈部肿瘤的缓解率较高,而毒性相对较低。Objective： To observe the effects and side effects of docctaxel combined with oxaliplatin in treatment of advanced head and neck tumors. Methods： 52 patients with advanced head and neck cancer were randomly divided into A group （26 cases）and B group（26 cases）, A group accepted docetaxel 75mg/m2 intravenously on day 1, oxaliplatin 130 mg/m2 intravenously, day 1, every 3 weeks as 1 cycle, totally 2 to 4 cycles ; B group accepted cisplatin 25mg/m2 intravenously, d1 - d3 , fluorouracil 3.5g/m2 , continuous infusion with infusion pump for 5days, 3 weeks as 1 cycle, altogether 2 to 4 cycles. Effect was evaluated two weeks after treatment. Results： 52 cases were available for each patient completed at laset two cycles of chemotherapy. Efficacy of A group was 53.8% （ 14/ 26）, B group 42.3% （11/26） （P 〉0.05）. 1-year survival rate of A group was 42.3%, B group 30.8% （P 〉0.05）. Life quality of 50. 0% （13/26） of patients in A group improved while in B group 26. 9% （7/26）improved（ P 〈 0. 05 ）. There was no significant difference of the incidence of myelosuppression between two groups（P 〉 0. 05 ）. The gastrointestinal side effect of A group was lower than that of B group（P 〈 0. 05）. Conclusion： Docetaxel plus oxaliplatin regimen can get high rimission rate in the treatment of advanced head and neck tumors with mild toxity.
