摘  要:目的:探索结合三维激光扫描技术的仿真颌骨模型在口腔种植中的应用。方法:选取就诊于北京大学口腔医院第二门诊部要求种植修复的20例患者,进行口腔颌面锥形束CT(cone beam computed tomography,CBCT)扫描,将扫描后的数据与3D激光扫描数据进行匹配,形成计算机三维颌骨模型并通过三维快速成型机形成实物的三维颌骨模型。术者在此模型上进行术前设计和模拟,制作手术导板。完成种植修复。结果:20例患者中12例颌骨的锥形束CT数据与三维激光扫描数据匹配准确;制作出手术导板,并在手术导板引导下顺利完成手术及最终修复。3例CBCT数据与激光扫描模型配比后发现CBCT在后牙区有偏差,最大偏差1mm。5例患者缺牙区骨质形态与临床不符,骨缺损范围偏大。结论:结合激光扫描技术的仿真颌骨模型在牙列区有清晰外形同时真实的显示了颌骨形态,为医生提供了一个熟悉而真实的模拟空间。Objective: To create an accurate jaw model with three-dimensional laser scanning technology.Methods: Twenty patients implant denture in the second center of Peking University Stomatological Hospital were included in this study.All patients accepted comprehensive and careful examination of oral cone beam CT scanning and had full mouth dentition impression.The cone beam CT scanning data of each patient and 3D laser scanning data of patient's dentition plaster model matched with three-dimensional targeting computer software to form three-dimensional computer model of jaw.And the final three-dimensional jaw model was formed by three-dimensional rapid prototyping machine.Results: Twelve of twenty patients with jaw of cone-beam CT data and 3D laser scanning data matched accurately;Surgical and rehabilitation planning and surgical guides were well designed and manufactured in these models;Implant operation and the final restoration were successfully performed with the guiding of the operation guide.Matching of CBCT data with laser scan model found CBCT deviation in the posterior area in three of 20 patients with the maximum deviation of 1mm.There were five cases showing discrepancies of bone morphology between the models and the real clinical condition,and the bone defects were larger in the models.Conclusion: The accuracy of three-dimensional jaw model formed by combing CT scan data with laser scanning techniques has significantly improved.
