^(99m)Tc-HYNIC-TOC显像诊断肾上腺嗜铬细胞瘤的临床价值

摘　　要：目的评价锝[^(99m)Tc]肼基烟酰胺奥曲肽类似物(^(99m)Tc-HYNIC-TOC)生长抑素受体显像诊断和鉴别诊断肾上腺嗜铬细胞瘤的临床价值。方法 327例肾上腺肿块患者进行^(99m)Tc-HYNIC-TOC显像,其中肾上腺嗜铬细胞瘤111例,其他类型肿瘤216例。结果 327例患者中^(99m)Tc-HYNIC-TOC显像阳性86例,包括肾上腺嗜铬细胞瘤71例。^(99m)Tc-HYNIC-TOC显像诊断肾上腺嗜铬细胞瘤的敏感性、特异性、准确率、阳性预测值和阴性预测值分别是64.0%、93.1%、83.2%、82.6%和83.4%。结论 ^(99m)Tc-HYNI(-TOC显像对肾上腺嗜铬细胞瘤的定性诊断和鉴别诊断具有极高的临床价值。Objective To assess the differential diagnostic value of somatostatin receptor imaging with ^99mTc-HYNIC-TOC （^oomTc-hydrazinon-icotinyl-Tyr3-octreotide） in adrenal pheochro mocytoma. Methods 327 patients with documented adrenal lesions underwent ^99mTc-HYNIC-TOC imaging, including 111 adrenal pheochromocytoma and 216 other adrenal tumours. Results Positive findings were obtained in 86 of 327 patients, including 71 adrenal pheochromocytoma. The sensitivity, specificity, accuracy, positive predictive value and negative predictive value of ^99mTc-HYNIC-TOC imaging for adrenal pheochromocytoma were 64.0% ,93.1% ,83.2% ,82.6% and 83.4% respectively. Conclusion ^99mTc-HYNIC-TOC imaging is a highly accurate and helpful method for differentiating adrenal pheochromocytoma from other adrenal turnouts.
