Survivin与高危型HPV的相关性及其在宫颈上皮内瘤变和宫颈癌诊断预测中的意义

摘　　要：目的探讨Survivin与高危型人乳头瘤病毒(HR-HPV)的相关性及其在宫颈上皮内瘤变(CIN)和宫颈癌(SCC)诊断预测中的临床意义。方法对80例SCC、82例CIN(其中CINⅠ级15例,CINⅡ级27例,CINⅢ级40例),进行病变组织检测:(1)采用免疫组化(SP法)检测组织中Survivin的表达;(2)采用PCR进行HPV检测。并对Survivin与HR-HPV进行相关性分析。结果 (1)HR-HPV与CIN及SCC密切相关,Survivin的表达强度和CIN的等级有相关性(r=3.11,P<0.05),HR-HPV阳性率与SCC临床分期呈正相关(r=3.17,P<0.05)。(2)在SCC组织中Survivin和HR-HPV的阳性率分别为98.7%和83.8%,CIN组织分别为85.4%和51.2%,差异有统计学意义(P<0.05)。(3)Survivin和HR-HPV的阳性表达率明显相关(r=3.51,均P<0.05)。结论 Survivin可作为宫颈癌的早期标记物,可独立进行HR-HPV类型的预测,上调的Survivin表达量可以独立的预测宫颈病灶处HR-HPV。Objective To investigate the correlation between Survivin and high risk human papilloma virus（HRHPV）, and predict value of Survivin in cervical intraepithelial neoplasia（CIN） and squamous cervical cancer（SCC）.Methods 80 patients with SCC and 82 patients with CIN（including 15 CINⅠ,27 CINⅡand 40 CINⅢ） were underwent immuno histochemical staining with Survivin antibody and HPV testing using polymerase chain reaction.The correlation between Survivin and HR-HPV was analyzed.Results（1） HR-HPV were associated with CIN and SCC.Linear regression were existed between intensity of Survivin expression and CIN grade（r = 3.11,P〈0.05）,the positive rate of HR-HPV was correlated with clinical stage of SCC（r =3.17,P〈0.05）.（2）The expression rate of Survivin and HR-HPV was 98.7%and 83.8% in SCC.which was 85.4%and 51.2%in CIN respectively,there were significant difference between CIN and SCC（P〈0.05）.（3）There was significant difference between positive expression rate of Survivin and HR-HPV（r =3.51,P〈0.05）. Conclusion Survivin seems to be an early predictor of SCC.It can independently predict types of HR-HPV.Up regulation of Survivin expression was an independent predictor of HR-HPV in cervical lesions.
