摘 要:目的探讨该地区人群不同亚型人乳头瘤病毒(HPV)感染与宫颈病变的关系。方法采用PCR DNA反向斑点杂交技术,对1 039例门诊首诊人群的宫颈脱落细胞标本进行HPV基因诊断和分型,所得数据用SPSS 17.0统计软件进行统计分析。结果 1 039例标本中HPV整体感染率为32.1%,其中单重感染占阳性标本的75.4%,双重感染和多重感染分别占阳性标本的17.7%、6.9%。HPV感染在小于30岁年龄段感染率最高,随着年龄的增加,感染率降低。HPV感染率随着宫颈病变级别的增加逐渐上升(P<0.001),HPV16亚型随宫颈病变级别的增加感染率逐渐上升(P<0.001)。不同年龄段HPV多重感染率差异有统计学意义(P<0.05)。结论 DNA杂交技术能一次性检测23种HPV型别,敏感性高、特异性强、操作简便、结果易于判读,适合临床应用。HPV分型检测对筛选宫颈高度病变有重要意义。Objective To investigate the relationship between various sub-type human papillomavirus(HPV) infection and path ological changes of cervix. Methods Cervix exfoliative cells from 1 039 cases of preliminarily diagnostic individuals were detected for HPV genotypes by using DNA hybridization. The data was analyzed with the statistical software SPSS 17.0. Results In total 1 039 cases,the positive rate of HPV DNA was 32.1%. In the HPV DNA positive cases,the rates of simple,double and multiple infection were 75.4 %, 17.7 % and 6.9 %. In different age groups, the HPV infection rate of 30 years old group was the highest, and there was a trend of the decrease of infection rate with the increase of age. HPV infection rate and the infection rate of HPV16 in creased with the increase of the grade of the cervix lesions(P〈0. 001). There was statistical difference of the multiple HPV infection rate between the different age groups(P〈0.05). Conclusion DNA hybridization can be used to simultaneously detect and clas sify different HPV genotypes with high sensitivities and specificities,which may be performed as a screen method to diagnose highgrade cervical lesion.
