宫颈癌调强适形放射治疗的疗效分析

摘　　要：目的:分析调强适形放射治疗宫颈癌的临床疗效。方法:将42例宫颈癌患者,随机分为治疗组和对照组,治疗组21例为调强适形放射治疗,对照组患者21例为192Ir高剂量率腔内加体外放射治疗。结果:治疗组近期总有效率100%,对照组90.5%,两组比较差异无统计学意义(P>0.05)。直肠放射性早期反应发生率治疗组为23.8%,对照组为76.2%;远期并发症发生率治疗组为9.5%,对照组为52.3%。膀胱放射性早期反应发生率治疗组为19.1%,对照组为61.9%,远期并发症发生率治疗组为4.7%,对照组为36.4%;治疗组和对照组比较差异有统计学意义(P<0.05)。结论:调强适形放射治疗宫颈癌可以取得与常规治疗相似的局控率,但能明显降低放射反应,避免了繁杂的腔内治疗操作,提高了患者的生存质量,具有明显优势。Objective To analyze the clinical curative effect of intensity-modulated conformal radiotherapy of cervical cancer.Methods 42 patients with cervical cancer patients,were randomly divided into treatment group and control group.In the treatment group,21 cases received intensity-modulated conformal radiotherapy,while in the control group,21 cases received 192Ir high dose in cervix and in vitro radiation therapy.Results Recently the total effective rate in treatment group was 100%,and the control group was 90.5% and there was no statistically significance between two groups（P0.05）.Rectum radioactive early reaction rate in treatment group was 23.8% group and 76.2% in control group;Long-term complication rate in treatment group was 9.5% group and 52.3% in control group.Bladder radioactive early reaction rate in treatment group was 19.1%,and the control group is 61.9%.Long-term complications rate in treatment group was 4.7 percent,36.4% in control group;There was a statistical significance between treatment group and control group（P0.05）.Conclusion Similar local control can be obtained in treating cervical cancer by applying intensity-modulated conformal radiotherapy and conventional treatment.But intensity-modulated conformal radiotherapy can obviously reduce radiation reaction and avoid the multifarious cavity operation,the advantage of which is obviously in improving patient＇s survival rate.
