疏肝健脾中药治疗功能性消化不良临床研究  

Recipe for Soothing Liver-qi and Invigorating Spleen-qi in Treatment of Functional Dyspepsia: A Clinical Trial

摘　　要：目的观察疏肝健脾中药治疗功能性消化不良(functional dyspepsia,FD)的疗效并探讨其作用机制。方法将100例FD患者随机分为对照组和治疗组,分别采用多潘立酮和疏肝健脾中药治疗,观察并比较两组临床疗效。检测FD肝郁脾虚证患者治疗前后血浆血管活性肠肽(vasoactive intestinal peptide,VIP)、P物质(substance P,SP)含量的变化,并与健康对照者比较。结果治疗组临床总有效率及胃脘胀满、胃脘疼痛、上腹灼热感、早饱的治愈率显著高于对照组(P<0.01)。与健康对照组比较,治疗前FD患者血浆VIP含量显著升高(P<0.01),血浆SP含量显著降低(P<0.05),治疗后VIP含量显著降低(P<0.01),SP含量显著升高(P<0.05)。结论疏肝健脾方药是治疗FD的有效方剂之一,疏肝健脾方药可以降低肝郁脾虚证FD患者血浆VIP含量,增加血浆SP含量。Objective To investigate into the therapeutic effects of Shugan Jianpi Decoction,a recipe for soothing liver-qi and invigorating spleen-qi,in treating functional dyspepsia（FD）,and explore its mechanism.Methods A total of 100 patients with FD were randomized into control group and treatment group,patients in the control group and the treatment group were treated with Domperidone and Shugan Jianpi Decoction,respectively.The therapeutic effects were observed and compared between the two groups.Before and after treatment,the plasma levels of vasoactive intestinal peptide（VIP） and substance P（SP） were assayed in 15 FD patients with liver-qi stagnation and spleen-qi deficiency and compared with those of other 15 healthy volunteers.Results The total effective rate,and the recuperation rates of upper abdominal distention,upper abdominal pain,upper abdominal burning sensation and early fullness in the treatment group were significantly higher than those in the control group（P〈0.01）.Compared with those of healthy controls,the plasma levels of VIP in FD patients were significantly increased（P〈0.01）,and the plasma levels of SP were significantly decreased（P〈0.05）;after treatment,the plasma levels of VIP in FD patients were significantly lower than those after treatment（P〈0.01）,and the levels of SP were significantly higher than those before treatment（P〈0.05）.Conclusion As one of effective recipes for FD,Shugan Jianpi Decoction can decrease the plasma level of VIP and increase the plasma level of SP.
