伤寒误下辨析

摘　　要：目的:辨析伤寒误用下法的体会,以指导临床应用。方法:对《伤寒论》中关于误用下法的条文进行整理、归纳、分析。结果:所谓误下,是指不当下而误下,或当下而太过,或当下而不足,或当以寒下而用温药,当以温下而用寒药,或当缓下而用急攻,或当急攻而用缓下,这些误下方式、程度等的差异,加之患者病情、体质的不同,可致气机升降开阖以及气血阴阳的损伤,导致出现千变万化的证候。故伤寒误下以后,不可简单认为"误下伤阴"或"误下伤阳",应根据误下后具体症状,辨证论治。结论:通过对伤寒误用下发的辨析,有利于临床的诊断和治疗,对临床有极大的指导作用。Objective：To analyze the misusing experience of purgation method in Shanghan Lun in order to guide clinical application.Methods：The provisions related with the misuse of purgation method in Shanghan Lun were sorted,classified and analyzed.Results：The so-called misuse of purgation method refers to the administration of purgation method when it should not be given,or the abusive or inadequate use of this method when it should be given,or the use of drugs with warm nature when the purgation method should adopt drugs of cold nature,or the use of drugs with cold nature when the purgation method should adopt drugs of warm nature,or the use of drugs with warm nature when the purgation method should adopt drugs of cold nature,or the use of eccoprotica when it should use cathartic,or the use of cathartic when it should use eccoprotica.These misused purgation methods with different manners and degrees,combined with various diseases and constitutions of the patients would lead to the disharmony of qi movement,such as the ascending,descending,opening and closing,as well as the injuries of qi,blood,yin and yang,resulting in ever-changing syndromes.Therefore,it is not simply that yin is damaged or yang is damaged after the misuse of purgation method.The syndrome differentiation and treatment should be conducted based on the specific symptoms after the misuse of purgation method.Conclusion：The analysis on the misuse of purgation method in Shanghan Lun is beneficial to clinical diagnosis and treatment with great clinical guiding significance.
