摘  要:目的探讨成人急性淋巴细胞白血病(ALL)患者外周血单个核细胞(PBMC)内两种糖皮质激素受体(GR)α和β表达水平与糖皮质激素(GC)治疗效应之间的关系。方法根据成人急性淋巴细胞白血病(ALL)患者对糖皮质激素治疗的反应,分为激素敏感组和激素抵抗组,同时选择健康志愿者作为对照组。采用半定量逆转录多聚酶链反应(RT—PCR)、免疫细胞化学方法分别检测两组成人ALL患者及对照组外周血单个核细胞中GRα和GRβ mRNA表达和蛋白的表达。结果①成人ALL患者激素敏感组、激素抵抗组和正常对照组均有GRα和GRβ的表达,但以GRα表达为主,其GRα/GAPDH mRNA分别为(1.13±0.67),(1.53±0.59)和(1.25±0.62);②激素抵抗型成人ALL患者体内PBMC中GRB mRNA表达增高,与激素敏感组和正常对照组相比(P〈0.05),差异有统计学意义,其GRβ/GAPDH mRNA分别为(1.48±0.55),(0.83±0.61)和(0.65±0.57);③免疫细胞化学结果显示,激素抵抗型成人ALLGR8阳性细胞数与激素敏感组和正常对照组相比(P〈0.01),差异有统计学意义,分别为(23.2±8.4),(5.6±3.3)和(6.1±4.5)。结论GR8亚型的高表达可能是成人ALL患者糖皮质激素抵抗的重要的预测因素之一。Objective To clarify the relationship between the expression of α and β isoforms of glucocorticoid receptors (GR) in peripheral blood mononuelear cells(PBMC) of patients with acute lymphohlastic leukemia(ALL) in adult and the response to glueocortieoid therapy. Methods Semi-quantitative RT-PCR was used to detect the expression of GR α and β in PBMC of patients with ALL and to observe the different responses to glucocorticoid. Immunocytochemical assay was used to detect the expression of protein of GRα and GHβ. Results (1)The expression of GRα mRNA was detected in a sensitive group and a resistant group of patients with ALL and all the controls with GHα/GAPDH mRNA ( 1. 13 ±0. 67 ), ( 1.53 ±0.59), ( 1.25 ±0.62 ) respectively. (2)The expression of GRβ mRNA in PBMC in the resistant group of patients with ALL was significantly higher than that in the sensitive group and the controls(P 〈 0.05) , with GRβ/GAPDH mRNA (1.48 ± 0. 55) , (0.83 ±0.61 ) and (0.65 ±0.57) respectively. (3)The number of GHβ positive PBMC in the resistant group was significantly higher than that in the sensitive group and the controls( P 〈 0.01 ) , with the number of GRβ positive PBMC (23.2 ±8.4 ), (5.6 ±3.3 ) and (6.1 ±4.5 ) respectively. Conclusion It is shown that the excessive expression of GRβ may serve as a novel predictor of glucocortieoid resistance in patients with ALL in abult.
