PA-MSHA辅助治疗晚期胃癌对外周血T细胞及NK细胞影响的研究

摘　　要：目的观察PA-MSHA辅助奥沙利铂联合ELF(足叶乙甙+亚叶酸钙+5-氟尿嘧啶)方案治疗晚期胃癌对外周血T细胞及NK细胞的影响。方法选择晚期胃腺癌患者40例及健康体检者20例,将患者随机均分为单纯化疗组和PA-MSHA联合化疗组。各组均使用奥沙利铂联合ELF化疗方案。PA-MSHA联合治疗组于术后同时使用PA-MSHA辅助化疗方案。抽取各病例外周抗凝血,用流式细胞仪检测并比较外周血T细胞及NK细胞的绝对数。结果术后化疗前单纯化疗组CD4+、CD4+/CD8+和NK细胞水平明显低于健康对照组及PA-MSHA辅助化疗组(P<0.01),PA-MHSA辅助化疗组CD4+、CD4+/CD8+和NK细胞水平低于健康对照组,但无统计学差异。第2个化疗周期开始前单纯化疗组CD4+、CD4+/CD8+和NK细胞水平明显低于PA-MSHA辅助化疗组(P<0.01)。结论 PA-MSHA辅助奥沙利铂联合ELF方案可增强晚期胃癌术后机体T细胞及NK细胞免疫功能,使晚期胃癌术后机体细胞免疫受损状态较早恢复,减轻化疗期间化疗药物对细胞免疫抑制作用。Objective To explore the clinical effect of PA-MSHA assisted Oxaliplatin combined with ELF（etoposide＋leucovorin ＋ 5-fluorouracil） regimen in the treatment of advanced gastric cancer.Methods Forty cases of advanced gastric adenocarcinoma confirmed by pathology and 20 cases of healthy persons were selected in our trail.All patients were randomly divided into chemotherapy group and chemotherapy with PA-MSHA.All patients received Oxaliplatin combined with ELF regimen.PA-MSHA treatment group was given PA-MSHA adjuvant chemotherapy,Vein blood samples were collected and absolute number of T cells and NK cells in the peripheral blood was measured.Results The absolute number of CD4＋,CD4＋/CD8＋ and NK cell were lower than those of PA-MSHA combined group（P0.05） and healthy group（P0.01） before chemotherapy.At the same time,the absolute number of CD4＋,CD4＋/CD8＋ and NK cell in PA-MSHA combined group were also lower than healthy group before chemotherapy,but there was no significant difference（P0.05）.The absolute number of CD4＋,CD4＋/CD8＋ and NK cell in group of chemotherapy were lower than that of PA-MSHA combined group（P0.01）.Conclusion PA-MSHA assisted Oxaliplatin combined with ELF scheme can enhance immune function of the T cell and NK cell in patients with advanced gastric tumor,make impaired cellular immune status of advanced gastric cancer patients recover earlier,and alleviate the immunosuppressive caused by chemotherapy.
