摘　　要：目的探讨保留皮肤的乳腺癌改良根治术后Ⅰ期硅胶假体植入再造乳房的并发症及处理方法。方法 2005年8月～2010年8月64例患者行保留皮肤的乳腺癌改良根治术后Ⅰ期于胸大肌后植入硅胶假体,即时再造乳房,根治术时根据冰冻结果决定是否保留乳头乳晕复合体。结果 64例患者行单侧或双侧保留皮肤的乳腺癌改良根治术+Ⅰ期假体植入再造乳房,1例患者出现包膜挛缩;1例患者出现皮瓣坏死,假体外露,予以皮瓣转移;3例出现皮瓣下积液,予以穿刺抽吸;2例出现假体移位;8例患者因对侧乳房下垂致外观不对称。结论严格掌握手术适应证及术中精细操作可以减少并发症的发生概率。Objective To discuss the treatment of complications of breast reconstruction with silica-gel implant immediately after skin-sparing modified radical mastectomy.Methods Sixty-four patients had their breast rebuilt by implanting silica-gel immediately after modified radical mastectomy from August,2005 to August,2010.Whether or not the nipple-areola complexes were preserved depended on the freeze pathological examinations.Results Sixty-four patients undertook single or both sides skin-sparing modified radical mastectomy and silica-gel implantation at the same time.One case suffered capsular contraction;one case suffered flap necrosis;three cases suffered under-flap hydration;two cases suffered gel metastasis and eight cases suffered obvious asymmetry.Conclusion Mastering the indication severely and operating technically may avoid the risk of complications.
