精制去纤酶治疗脑梗塞的疗效及相关因素分析

摘　　要：目的：观察精制去纤酶治疗脑梗塞的疗效，并探讨影响疗效的有关因素。方法：５０ 例病程在３～７２ ｈ 内脑梗塞静脉滴注精制去纤酶，首剂２ Ｂｕ，第３、５、７ ｄ 各用１ Ｂｕ；根据临床及实验室资料对其疗效进行多因素分析。结果：总有效率７８％ ，在２４ ｈ 内治疗的有效率为８１．９％ ，与２５～７２ ｈ 内治疗的疗效相比有显著性差异（ Ｐ＜ ０．０５），治疗后血纤维蛋白原、血浆粘度明显降低。多因素分析表明：疗效除与治疗时间有关外，还与病情程度、血糖水平有关，与年龄、性别、入院时血压、血脂水平等因素无关。结论：精制去纤酶治疗用于病情轻—中度，尤其是伴高纤维蛋白原血症的脑梗塞患者，应用越早，效果越明显。Objective:To evaluate the curative effect of defibrase in the treatment of cerebral infarction(CI) and to make a study of the factors concerned.Methods:50 patients received Defibrase intravenously within 3 to 72 hours after CI, the first dose being 2Bu, and 1 Bu for the following 3rd,5th,7th day,the correlative factors of effect were analyzed base on the clinical and laboratory information obtained.Results:The total effective rate is 78%.The 24-hour rate is 81.9%, better than those achieved within 25 to 72 hours after CI(P<0.05).The plasma Viscosity decreased significantly, compared to the preoperative condition. The effect was found to be associated with the treatment period and the levels of blood sugar and neurological deficit, but not with the age, sex, blood pressure,or levels of blood lipid on the admission.Conclusion:Defibrase may be applied to patients with mild and moderate CI.In those with complicated hyperfibrinogen, the earlier the treatment begins, the more the patient may benefit.
