经单鼻孔蝶窦入路显微手术切除垂体腺瘤并发症观察及护理

摘　　要：目的:探讨经单鼻孔蝶窦入路显微手术切除垂体腺瘤并发症观察及护理方法,降低并发症发生率。方法:2008年1月至2009年12月,经单鼻孔蝶窦入路显微手术切除垂体腺瘤50例病人,取合适的卧位,严密观察意识、瞳孔、生命体证,重视视力视野变化,记录每小时尿量及24h尿量,监测尿比重、电解质,观察鼻腔分泌物量、性质,观察颅内感染症状,面部感觉,做好舒适护理。结果:经单鼻孔蝶窦入路显微手术切除垂体腺瘤并发症明显减少,住院时间缩短。结论:经单鼻孔蝶窦入路显微手术切除垂体腺瘤创伤小,安全,出血少,术后患者痛苦小,恢复快,复发率低,患者舒适度高。Objective：We probe into the methods of observing and nursing in the complication of the microsurgery cutting off pituitary adenomas through the approach of the sphenoid sinus in the single nostril so as to reduce the morbidity of the complication.Method：Fifty patients took the microsurgery were observed from January 2008 to December 2009,we observed the consciousness,the papilla and the biotic signs of the patients in suitable decubitus.We payed attention to the changes of patients’ eyesight,and recorded the volume of patients’ urine per hour and per day in order to monitor the proportion of the urine and the electrolyte.We took good care of the patients by observing the amount and the nature of the mycteric secretion,observed the symptoms of encranial infect and the prosopo-sensation.Result：Adenomas through the approach of the forehead,the complications was reduced obviously,and the hospitalization could be cut down.Conclusion：This kind of microsurgery is much safer and characterized by a slighter wound,a mild hemorrhage,a faster recovery,a lower recurrence rate,and a higher degree of comfort.
