经前会阴入路治疗超低位直肠癌保肛手术体会

摘　　要：目的探讨经前会阴直肠癌切除术的临床效果。方法对12例确诊为直肠癌患者实施经前会阴直肠癌切除术,并观察治疗效果。结果无手术死亡病例,所有患者均获得5年随访,5年存活10例,术后1年,过半数患者能较好控制排便。术后1年,总体生存质量评分达75分以上,高于一般经腹会阴联合切除术患者([63.4±3.1)分,P<0.05)]。结论 APPEAR术能较彻底切除肿瘤组织,有效保留括约肌功能,在治疗超低位直肠癌时是一种可供选择的方法。Objective To explore the clinical effect of APPEAR（anterior perineal plane for ultra-lowanterior resection of therectum） . Methods 12 case with ultra-low rectal cancer were given APPEAR,and the effects were observed. Results No one died during the operation. and all the patients had been followed up for more than 5 years..10 patients were still alive five years after the operation. More than half of them can control the stool to a satisfying extent 1 year after the operation.And the total life quality scored to more than 75 one year after the operation,and this score was higher than that of the patients who undergo the Mile＇s operation（P0.05） . Conclusion APPEAR can excise the tumor thoroughly and reserve the function of anal sphincter,and APPEAR can be an option for ultra-low rectal cancer patient.
