7996食管癌患者凝血功能检测及临床意义探讨

摘　　要：目的许多研究均证实了恶性肿瘤患者经常出现止凝血参数异常,易于发生动脉或静脉血栓栓塞。本文通过对食管癌患者凝血功能检测的观察,探讨此检测结果变化的临床意义。方法采用德国美创1800自动血凝分析仪测定108例食管癌患者和80例健康体检者的凝血酶原时间（PT）、活化部分凝血活酶时间（APTT）、凝血酶时间（TT）、纤维蛋白原（Fib）,同时计数血小板（PLT）。结果食管癌组与正常组比较PT、TT检测结果无显著性差异（P〉0.05）,APTT结果较正常组延长,有显著性差异（P〈0.05）,Fib、PLT结果较正常组明显增高,有非常显著性差异（P〈0.01）。食管癌Ⅰ~Ⅲ期患者与Ⅳ期患者比较,凝血四项及PLT结果均无显著性差异P〉0.05）。结论 APTT结果的延长提示了食管癌患者存在凝血功能的紊乱,Fib和PLT的增高,提示食管癌患者较正常人群更易发生血栓,且其增高亦有利于恶性肿瘤患者的癌细胞发生转移。食管癌不同分期的患者的凝血功能均存在异常,无明显差异。因此,临床监测食管癌患者的凝血指标变化对临床治疗有一定的指导意义。Objective Considerable evidences showed that malignant tumor patients often appeared abnormal hemostatic and clotting parameters,and thrombosis was a common complication in cancer patients.To explore the clinical significance of the coagulation function of esophageal carcinoma patients through the observation of the test of coagulation function. Methods Blood samples were obtained from 108 esophageal carcinoma patients and 80 normal persons to observe levels of PT,APTT,TT;Fib and PLT.Results PT,TT in esophageal carcinoma patients did not have significant change compare to normal group（P0.05.APTT were higher than that in normal group（P0.05）.And Fib,PLT were obviously higher than that in normal group（P0.01）.The five indexes did not change significantly compare to different TNM classification group（P0.05）.Conclusions Esophageal carcinoma patients have coagulation function disorders,and these patients are prone to come up thrombosis.The observation of coagulation function of esophageal carcinoma patients provides reliable information for clinical therapy.
