摘  要:目的探讨水分离技术在经侧裂岛叶手术治疗高血压壳核脑出血中的应用及体会。方法 2010年1月至2010年11月间11例高血压壳核脑出血患者,超早期采用经侧裂岛叶清除血肿,术中应用水分离技术解剖侧裂和清除血肿。结果 11例患者均术后24 h内复查CT,9例血肿清除完全,2例血肿清除大于95%,11例患者均随访3月以上,ADL评分为Ⅰ级2例,Ⅱ级6例,Ⅲ级3例。结论水分离技术是一种安全简便的分离技术,在经侧裂岛叶清除高血压壳核脑出血中能很好帮助解剖侧裂和清除血肿,且对脑组织损伤小,血肿清除彻底。Objective To investigate the effect of water jet dissection technique in surgical treatment of hypertensive putamina hemorrhage through transsylvian transinsular approach. Methods 11 patients with hypertensive putamina hemorrhage from January 2010 to November 2010 underwent the resection of hemorrhage through transsylvian transinsular approach by water jet dissection technique. Results The postoperative CT within 24 h showed that hematoma was removed totally in 9 cases and over 95% in 2. Of 11 patients who were followed up for 3 months,2 belonged in grade I ,6 in grade II and 3 in grade m according to ADL. Conclusion The water jet dissection technique can be applied very simply and safely, which is very helpful for opening the sylvian fissure and removing hematoma on hypertensive putaminal hemorrhage through transsylvian transinsular approach with minimal injury of brain tissue and total removement of hematoma.
