正畸治疗技术在上下颌骨骨折中的创新应用

摘　　要：目的探讨上下颌骨骨折的正畸治疗替代疗法。方法选取颌骨骨折病例,根据每个病例的临床实际特点设计并制作不同的矫治器。治疗乳牙期、替牙期、恒牙期上颌骨复合骨折多用上颌活动矫治器配合口外前方牵引器、上颌螺旋缩弓器、弹性颅颌绷带联合牵引等活动矫治器;治疗下颌骨髁状突骨折、下颌骨多发性骨折所致咬关系不良多用固定矫治器配合颌间牵引。结果采用正畸矫治器的非手术方法治疗与固定上下颌骨骨折后,上下颌骨骨折断端复位愈合、牙弓三维形态复原、患者的咬功能得到最大程度恢复,正畸治疗达到了颌骨骨折手术治疗的疗效。结论在治疗上下颌骨骨折中正畸方法可以替代传统手术治疗方法,正畸治疗可以使颌骨骨折精确地愈合到功能位,同时恢复正常咬关系。Objective To Use a non-surgical technology to treat the upper and lower jaw fractures.Methods We select the clinical cases,design and produce different types of appliances according to their characteristics.Removable appliance including a screw device shrinkage of maxillary arch,active treatment combined with perioral maxillary protraction,elastic craniofacial bandages combined with traction was used to treat with the maxillary complex fracture in periods of the permanent and the deciduous teeth,while the fixed appliance combined with intermaxillary traction was used in malocclusions caused by fracuture of mandibular condyloid process and multiple fractures of the mandible.Results The non-surgical method of orthodontic appliances made the upper and lower jaw fractures fixed,the fracture sites reset and heal,the three-dimensional shape of the arch recover and ensured the occlusal function of patients with the greatest degree of recovery.Orthodontic treatment achieved the efficacy of surgical treatment of jaw fractures.Conclusion It is possible to treat the upper and lower jaw fractures with orthodontic method,which could ensure the mandibular fracture heal on the accurate functional position with normal occlusion.
