经皮经肝胆管穿刺引流术中胆管选择及穿刺方法分析

摘　　要：超声引导下PTBD40例分析胆管选择及穿刺方法。结果:40例患者38例成功,其中5例置管位置不理想;2例失败(其中1例因胆管内充满癌栓)。结论:(1)PTBD术胆管穿刺注重胆管、穿刺点、角度选择以及患者呼吸配合能够显著提高成功率及引流管置管质量。(2)低位胆道梗阻PTBD首选穿刺右肝管置管引流。We analyzed the biliary select and puncture method in 40 cases treated by percutaneous transhepatic biliary drainage(PTBD) .Result: PTBDs were success in 38 cases, failed in 2 cases and the situation of drainage tube of was not satisfaction in 5 cases. Conclusion: (1) success rate of PTBD can be obviously increased by paying attention to the biliary select、puncture dot、 puncture angle and respiration coordination of patients; (2) first select was right hepatic biliary in low situation obstruction of biliary.
