急性脑梗死患者血小板活化与血管内皮损伤标志物检测及其临床意义

摘　　要：目的探讨急性脑梗死患者血小板活化指标血小板ɑ颗粒膜糖蛋白(CD62P)、溶酶体膜蛋白(CD63)与血管内皮损伤指标血管性血友病因子(vWF)、血管性血友病因子裂解酶(vWF-cp)的变化及其临床意义。方法急性脑梗死合并高血压患者(ACI+HBP组)39例,单纯急性脑梗死患者(ACI组)16例,正常对照组50例,分别检测CD62P、CD63、vWF和vWF-cp的水平。结果 ACI组CD62P、CD63和vWF水平高于正常对照组(P<0.01),vWF-cp水平低于正常对照组(P<0.05);ACI+HBP组CD62P、CD63和vWF水平高于ACI组(P<0.01),vWF-cp水平差异无统计学意义(P>0.05)。结论 CD62P、CD63和vWF水平升高,vWF-cp活性降低可能与急性脑梗死的发生、发展、内皮损伤有关;急性脑梗死合并高血压患者,其血小板活化和血管内皮损伤程度更严重。Objective To study the clinical significance of plasma levels of CD62P,CD63,vWF and vWF-cp in patients with acute cerebral infarction.Methods 50 healthy people（control group）,39 patients with both acute cerebral infarction and high blood pressure（ACI＋HBP group） and 16 patients only with acute cerebral infarction（ACI group） were studied,the plasma levels of CD62P,CD63,vWF and vwF-cp were measured.Results The plasma levels of CD62P,CD63 and vwF in acute cerebral infarction patients were higher than those in healthy people（P0.01）,however the vwF-cp activity was lower（P0.05）.The levels of CD62P,CD63 and vWF in ACI＋HBP group were higher than those in ACI group（P0.01）,however the vWF-cp activity has no statistical significant difference（P0.05）.Conclusion The platelet activation and endothelial injury can contribute to the pathogenesis and development of acute cerebral infarction.And this physiopathologic course is more serious in patients with both acute cerebral infarction and high blood pressure.
