直肠癌患者淋巴结转移规律的相关因素分析

摘　　要：目的分析影响直肠癌患者淋巴结转移的相关因素,探讨其转移规律,为临床治疗、预后评估提供理论参考。方法选取性别、年龄、癌肿部位、大小、累及肠管周径、大体类型、组织类型、病理分级、浸润深度等临床病理因素,进行Kruskal-Wallis秩和检验、Logistic多因素回归分析,研究它们与淋巴结转移之间关系。结果单因素分析显示直肠癌患者淋巴结转移与其年龄、肿瘤大小、累及肠管周径、大体类型、组织类型、病理分级、浸润深度有关(P<0.05);多因素分析显示肿瘤大小、病理分级及浸润深度为影响淋巴结转移的主要因素(P<0.05)。结论影响直肠癌患者淋巴结转移的主要因素包括肿瘤大小、病理分级及浸润深度。Objective To analyze the factors related to lymph node metastasis in rectal cancer patients and explore the metastasis rule for clinical treatment and prognosis evaluation. Methods We used gender, age, tumor location, size, circumference of the intestine involved, gross type, histological type, pathologic grade and invasive depth for Kruskal-Wallis test and logistic regression analysis to analyze the relationship with lymph node metastasis. Results Univariate analysis data showed that lymph node metastasis in rectal cancer patients was related to their age, tumor size, circumference of the intestine involved, gross type, histological type, pathologic grade, and depth of invasion （P〈0.05）. Multivariate analysis results demonstrated that tumor size, pathologic grade, and depth of invasion were important factors affecting lymph node metastasis （P〈0.05）. Conclusion Some important factors affecting lymph node metastasis in rectal cancer patients include tumor size, pathologic grade, and depth of invasion.
