多发性骨髓瘤患者外周血moDC分泌APRIL和BAFF临床意义的研究

摘　　要：目的:探讨多发性骨髓瘤(MM)患者外周血单个核细胞来源树突状细胞(moDC)分泌的B细胞活化因子(BAFF)和增殖诱导配体(A-PRIL)的特点,明确树突状细胞(DC)在MM发病中所起的作用。方法:选取初诊(初诊组)MM患者18例(Ⅱ期8例,Ⅲ期10例),动态研究(动态组)MM患者8例,10名健康者为正常对照。ELISA法检测外周血清及moDC经LPS活化后上清的BAFF、APRIL含量,荧光实时定量PCR(RQ-PCR)测定活化后moDC BAFF、APRIL的表达。结果:MM患者外周血清及moDC培养上清BAFF、APRIL水平,moDC的BAFF、APRIL相对表达量除移植后外,各组均高于对照组;初诊组中Ⅲ期高于Ⅱ期,动态组中治疗前高于治疗后有效期以及自身造血干细胞移植后(移植后),有效期高于移植后。相关分析示,MM患者血清、moDC培养上清BAFF、APRIL,moDC的BAFF、APRILmRNA相对表达量之间分别有正相关关系。结论:DC分泌的APRIL和BAFF与MM发病及病情进展有密切关系,DC可能通过分泌APRIL、BAFF起重要作用。OBJECTIVE： To investigate the character of a proliferation-inducing ligand （APRIL） and B cell-activating factor（BAFF） secreted by peripheral blood monocyte-derived dendritic cell （DC） in multiple myeloma patients. METH- ODS： Eighteen MM patients with 8 stage]I and 10 stage HI （group untreated）, 8 MM patients （dynamic group）, and 10 health people （control group） were involved. APRII.,BAFF level in serum and the supernatant of peripheral monocyte derived DC （moDC） stimulated by LPS were tested with ELISA. APRIL, BAFF level in LPS-stimulated moDC were tested with RQ-PCR. RESULTS： The APRII. and BAFF level in serum and supernatant of moDC stimulated by I.PS and their mRNA contain in moDC in stage Ill were higher than those in stage ]~ , these factors in dynamic group before therapy were higher than after therapy. There were positive correlation among the APRIL, BAFF level in peripheral blood, the supernatant of moDC stimulated by I.PS and their mRNA contain in moDC in all MM patients. CONCLU- SIONS： APRII. and BAFF may contribute to disease development in MM. moDC may play an important role in MM pathogenesis through secreting APRIL and BAFF.
