平板型数字胃肠机在早期食管癌诊断中的价值

摘　　要：目的探讨平板型数字胃肠机在诊断早期食管癌中的价值。方法回顾性分析98例经手术、病理证实的早期食管癌患者的临床资料,根据X线检查机型分为平板型数字胃肠机组(dRF组,48例)和普通数字胃肠机组(R200组,50例),比较两组影像学资料。结果 dRF组48例中术前发现早期食管癌47例,漏诊1例,诊断敏感性为97.9%;R200组50例中发现早期食管癌43例,漏诊7例,诊断敏感性为86%。结论平板型数字胃肠机在很大程度上提高了影像清晰度、动态范围,扩大了视野,降低了患者受到的辐射量,提高了诊断早期食管癌的敏感性。Objective To explore the clinical value of the digital fluoroscopy system with flat panel detector in diagnosis of the early esophageal carcinoma.Methods Ninety-eight patients with surgically and pathologically proved early esophageal carcinoma were analyzed retrospectively.They were divided into two groups according to applied X-ray machines：the group of digital fluoroscopy system with flat panel detector（dRF group,n=48）and the group of common digital fluoroscopy system（R200 group,n=50）.To compare the differences of imging findings between the two groups.Results Among 48 cases of dRF group,47 cases were definitely diagnosed as early esophageal carcinoma while only one case were leaking-diagnosed,the sensitivity was 97.9%.Among 50 cases of R200 group,43 cases were definitely diagnosed and 7 cases were missed,the sensitivity was 86%.Conclusion The digital fluoroscopy system with flat panel detector improves imaging quality and enlarge dynamic range to a great extent.With the large flat detector,we can get a excellent coverage of the region and dose reduction for patients,and a brilliant sensitivity in diagnosis of the early esophageal carcinoma.
