食管癌根治性放射治疗联合放射增敏剂研究文献分析

摘　　要：目的了解放射增敏剂应用于食管癌放射治疗的国内临床研究文献的分布情况,并评价现有文献的质量。方法全面检索相关文献,并按照事先定义的纳入和排除标准剔除不符合要求的文献,对纳入文献采用文献计量学、Jadad评分等方法进行分析与评价。结果纳入的78篇文献中,仅有7篇文献标明有基金支持;甘氨双唑钠、氟尿嘧啶类和马蔺子素三类放射增敏剂的研究文献占总纳入文献数的54.4%;89.7%的文献采用常规分割放疗方法;75篇文献有近期疗效评价,但评定标准种类繁多,有远期疗效评价的文献仅有43篇;Jadad评分显示,低质量研究文献(≤2分)占总纳入文献数的80.8%,仅有13篇提到了具体的随机分配方法,69篇文献未采用盲法。结论目前放射增敏剂应用于食管癌放射治疗的国内临床研究偏少,基金支持力度不够,主要局限于几类放射增敏剂和常规照射方法的研究;现有文献主要为近期疗效研究,且评定标准不一,数据可比性差;现有研究的文献质量参差不齐,多为低质量文献,对临床的指导价值有限。Objective To systematically evaluate the distribution and quality of domestic clinical researches in esophageal cancer radiotherapy with or without radiosensitizer.Methods Bibliometrics,Jadad Scale were adopted to perform comprehensive quantitative analysis on the studies selected by predetermined eligibility and exclusion criteria.Results A total of 78 articles were evaluated,and 7 studies were funded.54.4% of the evaluated articles received CMNa,Fluorouracil,Irisquinone and their derivatives.89.7% of the evaluated researches received conventional fraction.75 articles had results for short-term clinical effect,and 43 for long-term clinical effect.Jadad score displayed that 80.8% of all studies selected were researches of low quality（≤2 points）.Only 13 articles mentioned randomizing ways,and 69 mentioned no blind technique.Conclusion So far,relevant researches were quite scarce and rarely funded in China.These researches mainly focused on several radiosensitizers and conventional radiotherapy.Assessment criteria of short-term clinical effect were varied in evaluated articles,which decreased the comparability of effect results.In addition,owing to low quality of most researches,the findings had less guideline value for clinical application.
