食管癌组织中p16蛋白表达及其临床意义

摘　　要：应用免疫组化ＳＡＢＣ法检测４９ 例食管鳞癌、１２ 例食管腺癌和２０ 例食管正常组织。结果显示，２８ 例食管鳞癌（５７．１％ ，２８／４９）４ 例食管腺癌（３３．３％ ，４／１２）和１８ 例食管正常组织（９０％ ，１８／２０）ｐ１６ 蛋白表达阳性（Ｐ＜ ０．０１），伴有淋巴结转移的食管鳞癌ｐ１６ 蛋白表达阳性率（３９．３％ ）显著低于无淋巴结转移者（８１．０％ ，Ｐ＜ ０．０１），中低分化食管癌ｐ１６ 蛋白表达阳性率（４６．９％ ）低于高分化者（７６．５％ ，Ｐ＜ ０．０５）。Ⅲ期食管癌ｐ１６ 蛋白表达阳性率（４４．４％ ）亦低于Ⅱ期者（７２．７％ ，Ｐ＜ ０．０５）。ｐ１６ 蛋白表达与肿瘤的大体病理类型无关。ｐ１６ 蛋白在食管癌中呈低表达，其表达阳性率随病期进展、淋巴结转移、细胞分化程度降低而明显降低，因此。To evaluate the p16 protein expression in esophageal carcinoma and the correlation between the expression and the clinical pathology, immunohistochemical assay(streptoavidin biotin peroxidase complex, SABC) was employed to detect the expression of p16 protein in 49 cases of esophageal squamous cell carcinoma (ESCC), 12 cases of esophageal adenocarcinoma and 20 cases of normal esophageal tissue. Positive immunostaining of p16 protein was found in 28 of 49(57.1%) with ESCC, 4 of 12(33.3%) with esophageal adenocarcinoma and 18 of 20(90%) normal esophageal tissue (P<0.01). The expression of p16 protein in the ESCC with lymph node metastasis was markedly lower than that without metastasis at positive rates of 39.3% and 81.0%(P<0.01). p16 protein was expressed in poorly and moderately differentiated ESCC at rate of 46.9%, lower than that of 76.5% in well differentiated ESCC(P<0.05). The p16 protein expression in the ESCC with stage Ⅲ was lower than that with stage Ⅱ at positive rate of 44.4% and 72.7%, respectively(P<0.05), and no correlation was found between p16 protein expression and pathologic and histologic types. The p16 protein expression was at low level in esophageal carcinoma. The positive rate of p16 protein expression was significantly decreased with the progression, metastasis and cell poor differentiation. The state of p16 protein expression can be regarded as a marker for the malignancy and prognosis of esophageal carcinoma.
