摘 要:采用嗜银染色、ABC免疫组化染色技术分别检测了60 例大肠癌组织中的AgNORs 颗粒数、AgNORs 面积、PCNA及p53 的表达量,结合临床Dukes′分期,采用Cox 回归分析评估AgNORs 颗粒数、AgNORs 面积、PCNA、p53 表达量及Dukes′分期在大肠癌预后评估中的作用。建立预后判别方程为pv =e((0 .710238Xl + 0.9185X2 + 0.527513X3 + 0.463156X4),判别值Ln(pv)≥12.0 时判别为存活5 a 以下,反之判为存活5 a 以上,灵敏度为90.3% ,特异度为93.1 % ,准确率为91 .6% 。提示AgNORs 计量、PCNA、p53 的表达多少可用于辅助评估大肠癌患者的预后。The eumeration and the area of AgNORs in the tissues of 60 cases of colorectal cancer were examined by argyrophilic staining techique.The expression of PCNA and p53 in above tissues were detected by immunohistochemistry method.A multivariate stepwise Cox's proportional hazard model analysis was performed to evaluate the prognostic value of the number of AgNORs,the area of AgNORs,the expression of PCNA,p53 and the Dukes′ grading.The formula was showed as fellowing.pv=e((0.710238X l+0.9185X 2+0.527513X 3+0.463156X 4),when Ln(pv)≥12.0,the patient was infered that he(she) couldn't survive over 5 year.When Ln(pv)<12.0,the patient was infered that he(she) could survive over 5 year.The efficienty was 91.6%,the sencitivity was 90.3%,the specificity was 93.1%.These results indicated that the AgNORs quantity and the expression of PCNA and p53 might be helpful to evaluate the prognosis of the patient with colorectal cancer.
