^(125)I粒子植入治疗中晚期胰腺癌的疗效观察

摘　　要：目的:观察125I粒子术中植入在中晚期胰腺癌治疗中的疗效。方法:将40例中晚期胰腺癌病人采用随机对照法分入粒子植入组和对照组,每组各20例。对照组,单纯行剖腹探查术;粒子植入组,术中肿瘤内植入125I粒子;两组病人中有梗阻症状者加行胆肠吻合术或胆道外引流术和(或)胃空肠吻合术。术前及术后1mo复查胰腺CT,测量肿瘤直径变化情况,判断疼痛程度,评定胰腺癌病人生活质量变化情况。结果:粒子植入组肿瘤直径有缩小趋势,腰痛腹痛明显改善,生活质量明显提高。对照组肿瘤直径无明显变化或增大,疼痛无明显改善;生活质量有所提高但无明显改善。两组结果有显著性差异(P<0.05)。结论:术中125I粒子植入治疗可明显抑制肿瘤生长甚至使肿瘤体积变小,可明显减轻、缓解Ⅲ期、Ⅳ期胰腺癌病人疼痛,从而提高病人生活质量。Objective： To observe curative effect of the ^125I particles implanted in patients with advanced pancreatic cancer in the treatment.Methods： 40 patients with advanced pancreatic cancer were randomized into implanted group and the control group.Every group has 20 patients.The patients by simple line laparotomy in control group.For particle implantation group,there were twenty patients implante ^125I seed when they get their laparotomy.Patients with obstructive symptoms plus choledochojejunostomy or external biliary drainage and（or） gastrojejunostomy anastomosis during the surgery.After one month,pancreas CT was used to measure changes in tumor size,to determine the degree of pain relief,and assess the quality of life changes.Results： For implantation group,tumor diameter tended to reduce,there was low back pain,abdominal pain,and the quality of life improved significantly.Control group has no significant change in tumor diameter,or even larger.There was no significant pain improvement,it improved the quality of life,but not significantly.Review of indicators compared between the two groups resulted in a significant difference（P〈0.05）.Conclusion： 125I seed implantation can significantly inhibit tumor growth,tumor size even smaller.Intraoperative 125I seed implantation therapy can significantly reduce、 ease stage Ⅲ,Ⅳ stage pancreatic cancer pain,to improve life quality in advanced pancreatic cancer.
