摘 要:目的:应用逆转录多聚酶链反应(RT-PCR)法检测肺癌外周血中的表达情况,并探讨其表达的临床意义。方法:用RT-PCR法检测98例肺癌患者外周血CK19 mRNA表达。结果:NSCLC组患者外周血CK19 mRNA的阳性率为77.55%(76/98),显著高于良性病变组的3.57%(1/28)和健康对照组的0,χ2=73.68,P<0.05;Ⅱ期〔87.50%(35/40)〕和Ⅲ期〔88.89%(24/27)〕NSCLC患者的阳性率显著高于Ⅰ期〔54.84%(17/31)〕患者的阳性率,χ2=13.453,P<0.05;淋巴结转移阳性和阴性患者的外周血中CK19 mRNA阳性率分别为89.55%(60/67)和51.61%(16/31),差异有统计学意义,χ2=17.523,P<0.05;而不同病理类型NSCLC患者外周血CK19mRNA阳性率之间差异无统计学意义,χ2=0.414,P>0.05。结论:肺癌患者外周血中CK19 mRNA表达较高可能与肺癌转移有关,检测CK19 mRNA对于早期判断患者转移和复发有一定的临床意义。OBJECTIVE:To evaluate the clinical significance of CK19 mRNA detection in the peripheral blood of patients with non-small cell lung cancer(NSCLC).METHODS: The expression of CK19mRNA was detected by reverse transcription polymerase chain reaction(RT-PCR) in the peripheral blood from 98 NSCLC patients.RESULTS: In the NSCLC group,the positive rate of CK19 mRNA in peripheral blood was 77.55%(76/98),which was significantly higher than that of the benign pulmonary lesion group 3.57%(1/28) and the healthy adults group 0(χ2=73.68,P〈0.05).The positive rate in stageⅠ,Ⅱ,Ⅲ NSCLC patients were 54.84%(17/31),87.50%(35/40) and 88.89%(24/27),respectively.The rate increased significantly as the clinical stage progressed(χ2=13.453,P〈0.05).No significant difference was noted between the positive rates of CK19 mRNA for different pathological types of NSCLC(χ2=0.414,P〉0.05).However,the positive rate of CK19 mRNA in patients with and without lymph node metastasis were 89.55%(60/67) and 51.61%(16/31),respectively.Significant difference was found for different lymph node metastasis status(χ2=17.523,P〈0.05).CONCLUSIONS: Higher expression of CK19 mRNA in peripheral blood of NSCLC patients may be related to the recurrence of the disease.A detection of CK19 mRNA is of definite significance for early prediction of NSCLC prognosis and metastasis.
