多层螺旋CT血管造影对面侧深区肿瘤诊断和血管定位的价值

摘　　要：目的探讨多层螺旋CT血管造影(multi-slice spiral computed tomography angiography,MSCTA)及图像后处理技术对面侧深区肿瘤的诊断价值和对毗邻血管定位的作用。方法分析19例我科住院诊治的面侧深区肿瘤患者的MSCTA,利用MRP、MIP、VR等图像后处理技术明确毗邻血管的移位情况以及其对肿瘤的诊断,结合手术中解剖情况进行分析。结果 19例患者中,MSCTA均能提供高分辨率的肿瘤实质影像,同时能显示肿瘤的边界及其与周围软组织和骨组织的关系及侵袭情况,对肿瘤性质判断具有较高的准确率。MSCTA强大的后处理技术能三维立体、任意角度展现肿瘤毗邻动脉的走行及其供血动脉,而且能确定血管与周围主要解剖标志的位置关系,为手术中对血管的处理提供准确的信息,增加了手术的安全性,血管的影像信息与术中所见一致。结论 MSCTA能准确的显示肿瘤实质的情况,同时能直观的显示肿瘤与毗邻大动脉的关系,能为手术中对血管的处理提供准确的信息和依据;其作为一种无创和经济的诊断方法,对面侧深区肿瘤的诊断和治疗具有重要意义。Objective To study the value of multi-slice spiral computed tomography angiography（MSCTA） and image postprocessing techniques to the diagnosis of tumors in the deep lateral facial region and to the location of the circumambient arteries.Methods MSCTA images of nineteen patients with neoplasm in the deep lateral facial region which had been cured in our department were analyzed retrospectively to identify the position of the surrounding arteries and the feeding arteries,and the diagnosis of the neoplasm through image postprocessing techniques,such as multi planar reformation（MPR）,maximum intensity projection（MIP） and volume rendering（VR）.The images and the intraoperative anatomy were compared.Results High spatial resolution images of MSCTA were obtained in the nineteen patients,and the images displayed the tumorous parenchyma,border,the circumambient bones,and soft tissues.Compared to pathological diagnosis,an excellent accuracy of the imaging diagnosis was achieved.The powerful image postprocessing technology of MSCTA uniquely provided three-dimensional anatomic information on the adjoining arteries and the feeding arteries in any angle and facilitated the comprehension of anatomic relationship among the tumor,arteries and bony landmark.This technique provided accurate information to the treatment of the arteries and increased the safety of operation.Conclusion MSCTA not only allows comprehensive assessment of the tumorous parenchyma but also provides important additional information about the surrounding carotid arteries and soft tissues,which is helpful to the intraoperative treatment of the arteries.As an economical and non-traumatic diagnostic method,MSCTA is valuable to the diagnosis and treatment of the tumors in the deep lateral facial region.
