摘　　要：目的报告1例腮腺区Merkel细胞癌并进行文献复习。方法对1例MCC患者的临床病史,肿瘤组织的病理免疫组化表达及治疗过程进行回顾性分析,并结合国内外已发表有关MCC临床表现、治疗及预后进行回顾性总结分析。结果本例MCC患者表现为颌面部皮肤Merkel细胞癌,同时伴发腮腺内肿瘤,病理表现均为小细胞恶性肿瘤浸润性生长,免疫组化显示CK20(+)、CK7(弱+);Syn(+);CgA(+);CD56(+)。结论 MCC极罕见且进展快,可能有局部伴发或复发肿瘤,检查时对同时发现两个肿瘤应有所警惕。发生于颌面部时应注意与粘液囊肿、涎腺肿瘤、恶性淋巴瘤和恶性黑素瘤等鉴别。免疫组化在该病的诊断和鉴别诊断中具有重要价值。Objective To investigate the diagnosis and differential diagnosis of the Merkel cell carcinoma（MCC）.Methods The clinical history,the immunohistochemical expression of the pathological tumor tissue,and the treatment course of 1 case of skin MCC were analyzed retrospectively.Combined with the literature on its clinical features,treatment and prognosis,the case was discussed.Results The MCC case showed oral maxillofacial MCC,accompanied by parotid gland tumor,both showing invasive growth of small cell tumors.Immunohistochemistry showed CK20（＋）,CK7（weak ＋）;Syn（＋）;CgA（＋）;CD56（＋）.Conclusion MCC,a rare and fast progress tumor,may be associated with localized tumors or local recurrence of tumor.Two kinds of tumors may be found at the same time.Maxillofacial MCC requires to be distinguished from Mucocele,salivary gland tumors,malignant lymphoma,and malignant melanoma.Immunohistochemistry is of great value in the diagnosis and differential diagnosis.
