空肠营养管在腹腔镜辅助手术中的应用

摘　　要：目的总结空肠营养管在大中型腹腔镜手术中的应用经验。方法 22例腹腔镜辅助腹腔脏器肿瘤切除术中预防性留置空肠营养管(EN组),术后24 h即开始进行肠内营养支持,32例腹腔镜术后未留置空肠营养管(PN组),术后早期行全胃肠外营养。比较两组胃肠功能恢复时间、手术并发症率、平均住院日以及平均住院费用。结果 EN组与PN组的肠蠕动恢复时间为(2.73±0.77)d vs.(3.25±1.04)d(P<0.05),平均住院日为(11.09±1.72)d vs.(14.25±1.83)d(P<0.01),平均住院费用为(37 427.25±3 354.29)元vs.(41 858.53±4 589.96)元(P<0.01),并发症发生率为4.5%(1/22)vs.25.0%(8/32)(P<0.05)。结论空肠营养管的应用可以早期恢复肠内营养支持,对于患者术后早期胃肠功能及肝功能的恢复和肺部感染、静脉炎等并发症的降低有明显的积极作用。Objective To summarize the experiences of the application of jejunum nutrition tube in the assisted laparoscopic surgery.Methods Laparoscopic jejunostomy was performed in 22 patients endured laparoscopic abdominal carcinoma resection.These patients included 13 cases of gastric carcinoma,5 cases of hepatocarcinoma and 4 cases of pancreatic neoplasm.Enteral nutrition（EN） was performed in 24 hours after operation.Another 32 cases of laparoscopic surgery received parenteral nutrition without the jejunal nutrition tube inputting（control group）.The time of bowel movement restoration,the complication rate,the average hospital stay and the average hospitalization expenses were observed.Results The time of bowel movement restoration in EN group was shorter than that of the control group（P〈0.05）.The average hospital stay,the complication rate and the average hospitalization expenses were reduced significantly（P〈0.01）.Conclusions The application of the jejunum nutrition tube could provide enteral nutrition support early after operation,and the jejunum nutrition tube plays an active character in recovering gastrointestinal function,lessening dysfunction of liver and reducing complication rates of pulmonary infection and phlebitis.
