丹参注射液治疗不完全性脊髓损伤后神经病理性疼痛的疗效观察

摘　　要：目的:探讨丹参注射液对不完全性脊髓损伤(ISCI)后不同性质的神经病理性疼痛的缓解作用。方法:共选取63例ISCI后神经病理性疼痛患者,随机分为丹参治疗组和按摩对照组,连续观测VAS评分、PPI评分在治疗第3、7、10天和随访时的变化以及SF-McGill评分在治疗前后和随访时的变化。结果:2组VAS、PPI评分在治疗第3、7、10天以及治疗结束后2周随访时差异有统计学意义(p<0.05);按脊髓损伤分级和脊髓损伤平面的进一步分析表明VAS、PPI评分变化差异依然具有统计学意义(p<0.05);丹参治疗组SF-McGill疼痛问卷跳动的、锐利的、热-烧灼的、触痛的等描述疼痛感觉类性质以及疲劳的筋疲力尽、惩罚的-令人痛苦等疼痛情绪类反应明显低于治疗前,差异有统计学意义(p<0.05);丹参治疗组疗效维持时间较按摩对照组长,差异有统计学意义(p<0.01)。结论:丹参能从感觉上和情绪上明显缓解ISCI后神经病理性疼痛;损伤程度对疗效有影响,程度轻的疗效好、持续时间也较长。Objective：To observe whether DanShen effective on neuropathic pain after ISCI and the affection of ISCI degree on DanShen＇effect.Methods：63 cases suffered from spinal cord incomplete injury were chose.They were divided randomly into two groups：（1） DanShen therapy group（n=32）;（2） Massage group（n=31）,VAS and PPI were evaluated before starting the therapy,during the 3,7,10day of treatment and two weeks following.SF-McGill was evaluated before starting the therapy,during the end of the treatment and two weeks following.Results：Both two groups＇scores of VAS,PPI were significantly lower than before treatment,the difference has significant respectively（p0.05）.The change of DanShen therapy group has significant difference according ASIA injury degree（p0.05）.After treated by DanShen,patients＇stab pain,sharp pain,burn pain,touch pain,fatigue to exhausted,punishing pain,among the selected questions were much lower than those before treatment,the differences were significant（p0.05）.DanShen group＇s time of maintain effectiveness was longer than Massage group＇s,difference was significant（p0.01）.Conclusion：DanShen can reduce the neupathic pain after incomplete ISCI.Spinal injury degree can affect DanShen＇effect,injury seriously cases had worse effect and shorter days of maintain effect.
