Metapex糊剂根管封药超填应用于根管再治疗的疗效观察

摘　　要：目的比较Metapex糊剂根管封药超填与氢氧化钙根管封药对牙髓治疗失败后根尖周病患牙行根管再治疗预后的影响。方法根尖周病患牙62颗行根管再治疗,分Metapex糊剂根管封药超填组32颗和氢氧化钙根管封药组30颗,随访分析比较2组的治疗效果。结果 Metapex糊剂根管封药超填组和氢氧化钙根管封药组再治疗成功率分别为93.7%(30/32)和83.3%(25/30),差异无统计学意义(P>0.05)。根尖周暗影小于或等于5 mm,Metapex糊剂根管封药超填组疗效优于氢氧化钙根管封药组,差异有统计学意义(χ2=6.574,P=0.037);根尖周暗影大于5 mm,氢氧化钙根管封药组疗效优于Metapex糊剂根管封药超填组(χ2=6.686,P=0.035);有窦道者,Metapex糊剂根管封药超填组疗效优于氢氧化钙根管封药组(χ2=4.971,P=0.043)。结论 Metapex糊剂超填是较适用于有窦道及根尖周暗影小于或等于5 mm患牙根管再治疗的根管封药方式。Objective To evaluate the efficiency of edodontical retreatment using Metapex extruding into periapical area and Ca（OH）2 within root canal in periapical lesion.Methods Sixty-two cases with periapical lesion which need retreatment were selected and followed up 1-3 years.Root canal retreation were used from 2007-2010 and all the cases were recalled and observed every three months.Difference of treatment effect between the two groups of Metapex extruding into the periapical and Ca（OH）2 in the root canal were compared.Results Successful rate of Metapex and Ca（OH）2 were 93.7%（30/32） and 83.3%（25/30）,respectively.No difference was found between the two groups.But with regard to the periapical lesion diameter ≤5 cm,diameter5 mm or the cases with fistula,significant difference was found between the two groups（P=0.000）.Conclusion To the periapical lesion were above 5 cm,the clinical outcome of Ca（OH）2 group was better than Metapex group;but to the periapical lesion were below 5 cm,the clinical outcome of Ca（OH）2 group was better than Metapex group;and to the cases with fistula,the clinical outcome of Ca（OH）2 group was better than Metapex group.Metapex extruding is efficient for the retreatment of periapical lesions with diameter less than 5 mm or fistulas,and will be recommended in clinic.
