益母草注射液在合并子宫肌瘤的终止妊娠术中及术后的应用

摘　　要：目的探讨益母草注射液在合并子宫肌瘤的终止妊娠术中的缩宫、止血作用。方法选择2009年1月至2010年6月本院门诊接受终止妊娠术合并子宫肌瘤患者中,符合本研究纳入标准和排除标准的53例患者为研究对象。将其按照配对分组法分为治疗组(n=27)和对照组(n=26)。两组术前子宫肌瘤直径、孕囊直径等比较,差异无显著意义(P>0.05)(本研究遵循的程序符合本院人体试验委员会所制定的伦理学标准,得到该委员会批准,分组征得受试对象本人的知情同意,并与之签署临床研究知情同意书)。治疗组术中予益母草注射液+缩宫素注射液肌内注射,术后继续予益母草注射液肌内注射3d;对照组术中仅予缩宫素注射液肌内注射,术后继续予缩宫素注射液肌内注射3d。观察两组术中缩宫素使用剂量,术中到术后2h出血总量,术后阴道流血时间,术后肌瘤直径缩小情况。结果两组术中缩宫素用量,术中至术后2h失血总量,术后阴道流血时间比较,差异有显著意义(P<0.05)。术后治疗组肌瘤直径稍大于对照组,但肌瘤直径均较术前缩小。两组组间术前、术后子宫肌瘤直径比较,差异无显著意义(P>0.05)。两组组内术前、术后子宫肌瘤直径比较,差异有显著意义(P<0.05)。两组术后子宫肌瘤直径缩小率比较,差异有显著意义(P<0.05)。结论益母草注射液能明显促进合并子宫肌瘤的终止妊娠术中、术后子宫的收缩,从而显著减少术中、术后出血量,且无刺激肌瘤生长等明显不良反应。Objective 1o investigate the uterine contraction and hemostatlc eltect oI motherwort injecta in induced termination of pregnancy surgery complicated with hysteromyoma. Methods From January 2009 to June 2010, 53 patients were divided into treatment group （n：27） and control group （n： 26）. Treatment group were given motherwort injectaq＋ oxytocin injecta during operation and continually given motherwort injecta for 3 days after termination of pregnancy surgery. Control group were merely given oxytocin injecta during and after the operation. To survey these targets on those 2 groups： the dose of oxytocin injecta used in operation; total amount of bleeding from beginning of the operation to 2 hours after the operation; the duration of the colporrhagia after termination of pregnancy surgery;the reduction diameter of the hysteromyoma. Results Compared with control group, treatment group had the lower dose of oxytocin injecta used in operation （P〈0.05） ; the less amount of bleeding during and 2 hours after operation （P〈0.05）, the shorter duration of the colporrhagia after termination of pregnancy surgery（P〈0.05） ; the diameter of the hysteromyoma in both group were reduced apparently after termination of pregnancy surgery （P〈0.05）, but the treatment group had the lower rate of the reduction of the hysteromyoma＇s diameter （P〈0.05）. Conclusion Motherwort injecta has a reliable effect on promoting the uterine contraction that can obviously reduce the amount of bleeding during and after termination of pregnancy surgery, and no apparent adverse effect has been detected such as stimulating the growth of the hysteromyoma.
