摘 要:目的探讨胆管癌组织中血管内皮生长因子和肿瘤微血管密度的表达及二者之间的关系。方法应用免疫组织化学方法测定51例胆管癌组织和10例正常胆管组织的血管内皮生长因子和微血管密度的表达水平。结果血管内皮生长因子在胆管癌组织中的阳性表达率为80.4﹪,在正常胆管组织中的阳性表达率为20﹪,两者相比差异有统计学意义(P<0.01);血管内皮生长因子在胆管癌组织中的表达与患者的肿瘤分化程度无明显相关性(P>0.05),而与有无淋巴结转移有相关性(P<0.05),转移组高于无转移组。肿瘤组的微血管密度计数明显高于对照组(P<0.01);微血管密度在胆管癌组织中的表达与患者的肿瘤分化程度无明显相关性(P>0.05),而与有无淋巴结转移有相关性,有淋巴结转移的胆管癌微血管密度值高于无淋巴结转移者(P<0.01)。胆管癌组织中血管内皮生长因子表达与微血管密度值呈明显正相关(P<0.05)。结论胆管癌存在肿瘤微血管的过度生成,血管内皮生长因子表达和微血管密度计数是反映胆管癌浸润、转移和预后的指标。Objective To investigate the expression of vascular endothelial growth factor (VEGF)and microvessel density (MVD) in cholangiocarcinoma and explore its relation with clinicobiological behavior. Methods Immunohistochemical technique was adopted in detecting the VEGF and MVD expression in cholangiocarcinoma and normal bile duct tissues. 51 cases of cholangiocarcinoma were collexted, and served as the test group, and 10 cases of normal bile duct tissues as control group. Results The expression rate of VEGF in cholangiocarcinoma was 80.4 %, whereas in normal bile duct tissue was 20 %, the difference has statistics meaning (P 〈 0.01). The expression of VEGF in cholangiocarcinoma was significantly associated with lymph node metastasis (P 〈 0.05), but not with tumor relativity (P 〉 0.05). VEGF expression of the patients with lymph node metastasis was higher than that of group without lymph node metastasis. There were significant difference in the levels of MVD between the patients with cholangiocarcinoma and controls(P 〈 0.01). The expression of MVD in cholangiocarcinoma was significantly associated with lymph node metastasis, but not with tumor relativity (P 〉 0.05). MVD counting value in patients with lymph node metastasis higher than in patients without lymth node metastsis (P 〈 0.01 ). The degree of VEGF expression had positive correlation with the MVD value in cholangiocarcinom tissues (P 〈 0.05). Conclusions Angiogenesis is developed in cholangiocarcinom. VEGF expression and MVD value are markers for the invasion, metastasis and prognosis of cholangiocarcinoma.
