危重症新生儿并发低血糖的营养支持护理

摘　　要：目的探讨危重症新生儿并发低血糖的营养支持护理方法。方法对86例并发低血糖的危重症新生儿在治疗原发疾病的基础上加强营养支持,同时动态监测血糖变化,根据血糖水平予早期经口喂养或鼻饲10%葡萄糖溶液、母乳或配方奶,或经输液泵静脉输注10%葡萄糖溶液或静脉营养液;严密观察病情变化、预防感染。结果患儿治疗后2-24 h血糖恢复正常,无一例发生低血糖性脑损伤;仅1例患儿发生喂养不耐受,均未发生坏死性小肠结肠炎及吸入性肺炎等喂养并发症。住院5-21 d出院。结论对并发低血糖的危重症新生儿加强营养支持,动态监测血糖变化,及时采取相应措施纠正低血糖并加强护理,可维持机体正常恒定的血糖水平,有利于患儿早日康复。Objective To explore nutritional supporting methods for critically ill neonates complicated with hypoglycemia.Methods A total of 86 critically ill neonates were complicated with hypoglycemia.They were given nutritional support based on treatment of primary diseases.Meanwhile,nurses monitored the fluctuation of blood sugar of neonates,fed them with 10% glucose,breast milk,or formula milk through oral route or a nasogastric tube,or administered 10% glucose or nutrition formula via a peripheral venous route.Nurses also closely observed neonates′ conditions,and took precaution to prevent infection.Results Blood sugar of the neonates turned normal within 2-24 h of nutritional support,with no cases of hypoglycemic brain injury.Only one neonate was into-lerant to feeding.The neonates had a hospital stay of 5-21 d without developing such feeding-associated complications as necrotic enterocolitis and aspiration pneumonia,etc.Conclusion For critically ill neonates complicated with hypoglycemia,we should monitor their blood sugar dynamically,take timely measures to correct hypoglycemia,and reinforce nursing care,which helps maintain normal blood sugar level,and boost early recovery.
