摘　　要：目的探讨意外胆囊癌的临床与病理学特征、诊断与鉴别诊断以及治疗原则和预后。方法回顾分析2007年7月～2010年9月诊断的9例意外胆囊癌临床与病理资料。结果肿瘤表现为息肉状突起或胆囊壁增厚变硬,镜下表现为异型腺上皮呈腺样或巢团状浸润性生长。免疫组化CK7(+),CK20(-),CEA(+),EMA(+)。中位随访期为26.7个月,有6例存活,3例死亡。结论意外胆囊癌由于症状无特异性,术前诊断困难。治疗应根据肿瘤的不同分期采取不同的手术方式。Objective Purpose To explore the clinicopathologic features, diferential diagnosis and treatment of unexpect- ed gallbladder cancer. Methods Nine cases of unexpected gallbladder cancers were studied from 2007 to 2010. Results The tumors were looked like polypi or gallbladder wall became thick and hard. Microscopically, the tumor tissue was made of gland and mass composed of atypia glandular epithelium, hnmunohistochemistry showed the tumor tissue was positive for CK7, CEA and EMA, and negative for CK20. The median follow-up time was 26.7 months. Six patients suvived and three died. Conclusion Unexpected gallbladder cancer is diffieuhly diagnosed before operation because without special symptom. Dift^rent treatment shall be adopted according to different tumor staging.
