血清透明质酸在梗阻性黄疸肝损伤模型中的变化及其意义

摘  要:目的研究血清透明质酸的改变在梗阻性黄疸肝损伤中的变化及意义。方法雄性Wistar大鼠60只,随机分为胆总管结扎组(n=30)和假手术组(n=30)。每组又根据术后不同时间点(1 d、3 d、7 d、14 d、21 d)分为5个小组(6只/组),分别采用放射免疫法、比色法、肝脏组织病理学检测等方法,观察各组动物体内血清透明质酸、肝功能、肝组织髓过氧化物酶、中性粒细胞聚集情况以及肝脏组织病理学变化。结果①胆总管结扎组各时间点所测得的血清透明质酸(serum hyaluronicacid,HA)、血清谷丙转氨酶(serum alanine aminotransferase,ALT)、血清总胆红素(total bilirubin,TBIL)均较假手术组明显升高(P<0.05)。②与假手术组相比,胆总管结扎组于术后3 d肝脏组织内中性粒细胞浸润明显增多(P<0.05)。胆总管结扎组内血清透明质酸的变化与肝脏组织内髓过氧化物酶(myeloperoxidase,MPO)含量、中性粒细胞聚集程度呈正相关性(rMPO=0.469,P<0.01;r中性粒细胞=0.398,P<0.05)。结论梗阻性黄疸过程中所出现的血清透明质酸增高与肝脏内所聚集的中性粒细胞对肝窦内皮细胞的损伤有关。Objective To investigate the changes of serum hyaluronic acid in liver injury rats of obstructive jaundice and its significance. Methods Sixty male Wistar rats were randomly divided into obstructive jaundice group(n=30) and sham operation group(n=30).Each group was divided into 5 subgroups according to the different days after operation.Tissues and blood samples were collected at days 1,3,7,14,21 after operation.Then level of serum hyaluronic acid was examined using radioimmunity assay.The liver function and level of myeloperoxidase in hepatic tissue were measured by colormetric assay.The accumulation of neutrophil cells and pathological changes were observed under microscopy after HE dye. Results ①Serum alanine aminotransferase(ALT),total bilirubin(TBIL),hyaluronic acid(HA) levels were all significantly higher in obstructive jaundice group than those in sham operation group at the same time point(P0.05).②The accumulation of neutrophil cells in liver tissues in obstructive jaundice group was significant higher than those in sham operation group at the same time point.The change of serum hyaluronic acid was positively correlated with the changes of myeloperoxidase(MPO) in hepatic tissue and accumulation degree of neutrophil cells in obstructive jaundice groups after 3 days of operation(rMPO=0.469,P0.01;rneutrophil=0.398,P0.05,respectively). Conclusion The increase of serum hyaluronic acid in obstructive jaundice may be associated with the injury of sinusoidal endothelial cells caused by accumulation of neutrophil cells in the liver.
