摘　　要：目的观察首次脑梗死患者血浆基质金属蛋白酶-9(matrixmetalloproteinase-9,MMP-9)的动态变化,探讨MMP-9在脑梗死发生发展过程中的意义。方法随访监测52例首次脑梗死患者发病2d内、2周、3个月及6个月以及20例健康体检者血浆MMP-9的含量。结果急性脑梗死发病2d内的血浆MMP-9水平显著高于发病2周后(P<0.01)和健康对照组(P<0.01),发病2周后血浆MMP-9水平高于健康对照组(P<0.05),发病3个月及6个月的血浆MMP-9水平与健康对照组比较差异无统计学意义(P>0.05)。神经功能缺损程度与脑梗死发病2d内血浆MMP-9水平呈正相关(r=0.769,P<0.01)。结论脑梗死急性期MMP-9水平升高,MMP-9水平可以反映急性脑梗死的病情严重程度。脑梗死恢复期MMP-9水平升高不明显,血浆MMP-9可能不能作为监测脑梗死复发前期的生化指标。Objective To explore the change of plasma MMP-9 in the first cerebral infarction and its significance.Methods Plasma MMP-9 levels were measured in 52 cerebral infarction patients（during the onset 2 days,2 weeks,3 months and 6 months） and 20 healthy people.The relationships between clinical neurologic deficit score and the level of plasma MMP-9 were analyzed.Results The level of MMP-9 at onset 2 days was conspicuously higher than that at onset 2 weeks（P0.01） and control group（P0.01）,the level of MMP-9 at onset 2 weeks was higher than control group（P0.05）,while there was no significant difference between the both latter groups（P0.05）.Conclusion The level of MMP-9 increases in the early stage of cerebral infarction,and is associated with the clinical course of the disease.Plasma MMP-9 cannot be the biochemical index of cerebral infarction recurrence.
