产前超声对胎儿肾脏畸形的诊断分析

摘　　要：目的探讨产前超声诊断胎儿肾脏畸形的声像图特征和超声诊断思维方法。方法广州市第十二人民医院2006年3月～2011年3月孕期超声检出82例胎儿肾脏畸形,对检出结果进行诊断分析。结果各种胎儿肾脏畸形的声像图特征不同,本组胎儿肾脏畸形82例,其中胎儿先天性肾缺如8例(其中双肾缺如4例,单肾缺如4例);2例异位肾;多囊肾10例中成人型多囊肾6例,婴儿型多囊肾4例;多囊性肾发育不良6例,重复肾6例,融合肾4例,肾外肾盂4例,肾积液24例,胎儿巨大输尿管并肾发育不良2例;肾囊肿16例中单发性肾囊肿12例,多发性肾囊肿4例。结论产前超声诊断胎儿肾脏畸形,准确率较高,超声依靠其特有的声像图改变及合并羊水量变化,对胎儿的临床诊断、预后判断及尽早采取措施提供声像依据,这对优生优育及新生儿早期治疗具有重要的临床价值。Objective To study the prenatal ultrasound diagnosis of fetal malformations of the kidney and ultrasound sonographic features of thinking.Methods 82 cases of fetal renal malformations were detected with pregnancy ultrasound in Guangzhou No.12 People＇s Hospital from March 2006 to March 2011,and the detection results were diagnozed and analyzed.Results The results of the sonographic fetal renal malformations showed different characteristics,this group had 82 cases of fetal renal abnormalities：congenital renal agenesis in 8 cases（renal agenesis in 4 cases,single renal agenesis in 2 cases）,one case of ectopic kidney;polycystic kidney disease in 10 cases：adult polycystic kidney disease in 6 cases,infantile polycystic kidney disease in 4 cases;polycystic renal dysplasia in 6 cases,repeated kidney in 6 cases,fusion of the kidney in 4 cases,renal pelvis in 4 cases,kidney effusion in 24 cases,fetal huge ureter complicated with renal dysplasia 2 cases;renal cysts 16 cases：solitary renal cysts in 12 cases,multiple renal cysts in 4 cases.Conclusion Prenatal ultrasound diagnosis of fetal renal malformations has a higher accuracy rate.With unique sonographic changes and merged changes in amniotic fluid,ultrasound provides audio-visual basis for clinical diagnosis,prognosis,and measures to take as soon as possible,which has significant clinical value for pr-and-post-natal care and early treatmentof newborns.
