显微手术治疗颅内动脉瘤322例预后影响因素分析

摘　　要：目的探讨影响显微手术治疗颅内动脉瘤预后的相关因素。方法总结我院自2006年6月～2009年5月收治的322例颅内动脉瘤显微手术病例资料,统计分析年龄、性别、血压、血糖、手术时机、Fisher's分级、Hunt-Hess分级、脑血管痉挛(cerebral vasospasm,CVS)、白细胞升高程度、感染等因素对患者预后的影响程度。结果 Hunt-Hess分级、Fisher's分级、CVS、年龄、高血压(基础病)、手术时机、白细胞升高程度、感染是影响此类患者预后的相关因素(P<0.01),Hunt-Hess分级、Fisher's分级与预后呈线性负相关关系。结论 CVS、Hunt-Hess分级、Fisher's分级、年龄、感染是影响预后的独立因素,CVS影响预后的危险度最高。【Objective】 To explore correlative prognostic factors of microsurgical treatment of intracranial aneurysms.【Methods】 Data of 322 cases of intracranial aneurysms receiving microsurgery from June 2006 to May 2009 were summarized.The influences of factors such as age,gender,blood pressure,blood sugar,operation time,Fisher＇s grading,Hunt-Hess grading,cerebral vasospasm（CVS）,the increased degree of WBC and infection on the prognosis of patients were statistically analyzed.【Results】 Hunt-Hess grading,Fisher ＇s grading,CVS,age,hypertension（foundation disease）,operation time,the increased degree of WBC and infection were the factors related to the outcomes of the patients（P〈0.01）.Hunt-Hess grading and Fisher＇s grading were in negative linear relationship with prognosis.【Conclusions】 CVS,Hunt-Hess grading,Fisher＇s grading,age and infection are the independent factors affecting the prognosis,among which CVS is of the highest risk.
