摘 要:目的:探讨多药耐药相关蛋白(MRP)基因表达与原发上皮性卵巢癌(PEOC)耐药的关系及其临床意义。方法:应用半定量RT-PCR技术,检测37份PEOC,10份正常卵巢上皮组织和20份正常女性外周血单个核细胞(PBMC)新鲜标本MRP基因的表达,用MTT法检测PEOC患者的原代癌细胞对临床常用抗癌药物的敏感性。结果:(1)PEOCMRP基因表达水平明显高于正常卵巢上皮组织(P<0.05),与PBMC差异无显著性(P>0.05);(2)MRP基因表达与PEOC临床期别及组织学类型无关,但随组织学级别升高而升高(P<0.05);(3)MRP基因高表达组MMC、ADM、E-ADM、VCR、VP16和CDDP对PEOC细胞抑制率与低表达组差异有显著性,余则无显著性;(4)MRP和MDR1基因表达里正相关(r=0.213,P<0.01),且与无进展生存期有关,而与总体生存期无关。结论:MRP基因表达与PEOC耐药有关,是抗MDR相关药物的重要原因之一。MRP基因表达的检测对临床用药有指导意义,对临床疗效判断和患者预后预测有一定价值。Objective To explore the association between the expression of multdrug resistance-associated protein (MRP)gene and the drug resistance of primary epithelial ovarian cnacer(PEOC),and to investigate the clinical significance of the expression of MRP gene in PEOC. Methods: Using RT--PCR assay, the expressions of MRP gene in 37 PEOC. 10 normal ovary tissues and 20 PBMC samples were examined. Sensitivity of the tumor tissues to the clinical common anticancer agents was examined using MTT assay. Results: (1)The expressions of MRP gene in PEOC were higher than those in normal ovarian tissues(P<0.05). But comparied with PBMC, there were no differences (P>0.05). (2) There were no correlations respectively betweer MRP gene expression and FIGO stage or histological type of PEOC. But the poorly the histological differentiation was, the higher the expression of MRP gene was. (3)PEOC tissues with MRP gene high expression were less sensitive to MMC. ADM. E-ADM. VCR. VP16 and CDDP. (4) Positive correlation was apparent between MRP and MDRI gene expression. Patients with MRP gene high expression tumors had a significantly shorter progression-free survival than those with MRP gene low expression. Conclusions: MRP gene expression related to resistance of PEOC are major causes of resistance to MDR related drug. Detecting the expression of MRP gene can help in selecting chemotherapeutic drugs and predicting the prognoses of patients.
