摘  要:目的研究外源性血管内皮生长因子对肝硬化大鼠门静脉压力的影响,以探索一种新的门静脉高压治疗方法。方法 25只肝硬化门静脉高压造模成功SD雄性大鼠,体质量180220 g,随机分为肝硬化门静脉高压对照组(B组,n=10)和肝硬化门静脉高压实验组(C组,n=15),C组应用Alzet微渗泵从大鼠门静脉泵入血管内皮生长因子165(30 ng/d)持续2周,正常对照组(A组,n=10)和B组开腹后关腹作对照,2周后对各组大鼠进行门静脉压力测定,光镜和电镜下观察肝组织病理学改变。结果 C组门静脉压力明显低于B组(P〈0.01)。光镜下观察C组肝细胞水肿及肝窦病变较B组减轻,电镜下观察B组肝窦内皮细胞形态不规则,窗孔缺失,基底膜形成,C组病变减轻。结论血管内皮生长因子能够降低肝硬化门静脉高压大鼠的门静脉压力。Objective To study the effect of portal vein administration of vascular endothelial growth factor in rats with liver cirrhosis to find a new therapy for portal hypertension.Method 25 male Sprague-Dawley rats with portal hypertension,weighing 180 to 220 g,were randomly divided into two groups:portal hypertension treatment group(group C,n =15) and portal hypertension control group(group B,n =10).Group C was given recombinant rat vascular endothelial growth factor 165(30 ng/d) via the portal vein for 2 weeks.Group B and normal control group(group A,n =10) underwent abdominal suture after laparotomy.After treatment,portal venous pressure was measured,hepatic histopathology was observed by light microscopy and ultrastructure of hepatic sinusoid was observed by transmission electron microscopy.Results Portal venous pressure in group C was obviously lower than group B(P 〈 0.01).Light microscopy fingdings showed that pathologic fibrotic changes had been improved in group C.Electron microscopy findings showed decreased number of fenestrations in endothelial cells of hepatic sinusoids,basement membrane formation,and irregular hepatic sinusoid endothelial cells in group B.These pathological changes were more severe in group B than group C.Conclusion Vascular endothelial growth factor is capable of lowering portal venous pressure in rats with liver cirrhosis.
