社区综合干预对分裂症患者药物消耗量和社会功能的影响

摘　　要：目的评价社区综合干预对分裂症患者精神症状,药物消耗量、社会功能的影响。方法选取社区精神分裂症患者200例,随机分为干预组100例,对照组100例,观察12个月。干预组给予社区综合干预,包括健康教育、社会功能锻炼、药物干预及心理干预;对照组仅作门诊随访不进行特殊干预。用阳性和阴性症状量表(PAN SS)、社会功能缺陷筛查量表(SDSS)于基线和干预后4,8,12个月末分别进行评定。并记录基线和4,8,12个月末前1周的用药量。结果干预组PAN SS总分(61.78±8.46)于第4月末起减分明显优于对照组(65.21±7.58)(t=3.02,P<0.01);干预8,12个月末,干预组SDSS总分及各因子分明显低于对照组(P<0.05或P<0.01);8个月末开始,干预组日均药物消耗量(467.61±63.97)明显低于对照组(497.21±60.02)(t=3.36,P<0.01)。结论社区综合干预可以改善精神分裂症患者的精神症状,降低药物消耗量,减少社会功能缺陷。Objective To evaluate the effect of community-based comprehensive intervention on mental symptoms,medicine consumption and social function of patients with schizophrenia.Methods A total of 200 cases who met the diagnostic criteria of CCMD-3 for schizophrenia were divided into intervention group（n=100）and control group（n=100） for an observation of 12 months.The intervention group was received community-based comprehensive intervention,including health education,social functional trainting,medicine intervention and psychological intervention,while the control group only for out-patient follow-up without special intervention.All subjects were evaluated with Positive and Negative Syndrome Scale（PANSS） and Social Disability Screening schedule（SDSS） at baseline and the end of 4,8,12 months.The doses of medicine in a week were recorded before base and the end of 4,8,12 months.Results The total score of PANSS in intervention group（61.78±8.46）was significantly reduced than that in control group（t=3.021,65.21±7.58）since the end of 4th months（P0.01）.The total score of SDSS and its factor scores in intervention group were significantly lower than those in control group at the end of 8,12 months（P0.05 or P0.01）.The average daily consumption of medicine in intervention group（467.61±63.97）was significantly lower than that in control group（497.21±60.62）form the end of 8th months（t=3.359,P0.01）.Conclusion Community-based comprehensive intervention can improve the mental symptoms of schizophrenia,reduce medicine consumption and decrease social disability.
