Logistic回归分析多生物指标在鼻腔鼻窦涎腺癌中表达的临床意义

摘　　要：目的:筛选出与鼻腔鼻窦涎腺癌密切相关的生物指标,并结合临床病例分析,以指导临床治疗和评估预后。方法:通过免疫组织化学法检测多种目前研究较为广泛的与恶性肿瘤转移复发相关的生物指标在鼻腔鼻窦涎腺癌、癌旁组织和对照组中的表达,并对其结果进行Logistic回归分析。结果:多分类的Logistic回归分析结果显示,在鼻腔鼻窦涎腺癌的发生过程中层黏连蛋白(LN)、Ⅳ型胶原蛋白(ColⅣ)及微血管密度(MVD)的表达有统计学意义(P<0.05),结合临床病例发现LN和ColⅣ共同阴性表达的患者更易发生复发和转移。结论:在鼻腔鼻窦涎腺癌发生过程中LN和ColⅣ是保护因素,而MVD则是危险因素。LN和ColⅣ共同减少与鼻腔鼻窦涎腺癌的复发转移密切相关。临床上可通过联合检测LN和ColⅣ以指导鼻腔鼻窦涎腺癌的治疗及预后评估。Objective To guide the clinical treatment and evaluate prognosis through screening several biological indicators related with nasal and sinusal salivary gland carcinoma and analyzing the clinical data.Methods The Logistic regression analysis was employed to analyze the data of the expressions of several biological indicators in nasal and sinusal salivary gland carcinoma,para-cancer tissues and nasal polys and cyst detected with immunohistochemical technique.Results There were significant differences in the expressions of laminin（LN） and type Ⅳcollagen（ColⅣ） and micrangium in the occurrence of the nasal and sinusal salivary gland carcinoma（P〈0.05）,and there was a poorly prognosis of nasal and sinusal salivary gland carcinoma if the levels of LN and ColⅣ reduced.Conclusion The LN and ColⅣare protective factors and the micrangium is a risk factor in the occurrence of the nasal and sinusal salivary gland carcinoma.The decreasing in LN and ColⅣexpressions have relationship with recurrence and metastasis of nasal and sinusal salivary gland carcinoma.The combined detection of LN and Col Ⅳ in nasal and sinusal salivary gland carcinoma can guide its clinical treatment and prognostic evaluation.
