摘 要:目的:研究唑来膦酸处理颗粒状植骨材料Bio-oss及种植体对植骨愈合和种植体骨结合的影响。方法:40只雌性SD大鼠随机分成3组:假手术组(A,n=15)、卵巢切除组(B,n=15)和唑来膦酸治疗组(C,n=10)。B、C组行双侧卵巢切除术。术后12周分别测量A组、B组大鼠股骨骨密度(BMD),组织学改变骨质疏松状态。在大鼠右侧胫骨近中干骺端植入羟基磷灰石涂层种植体,并于种植体旁制备骨缺损,Bio-oss同期植入术。C组种植体及Bio-oss在术前用唑来膦酸处理。种植术后第4、12周分2批处死动物,制作不脱钙标本切片,形态学观察及骨计量学检测。结果:卵巢切除组与假手术组比较股骨BMD显著降低(P<0.01),证实骨质疏松造模成功。种植术后4、12周,B组TCBI、BCR、NBV、ERBP均显著低于A组和C组(P<0.01);而C组除TCB显著低于A组外,其它指标均显著高于A组(P<0.01)。结论:实验性骨质疏松可延迟Bio-oss植骨愈合,降低种植体骨结合率;唑来膦酸局部应用可拮抗骨质疏松的影响,促进植骨愈合,增加种植体骨结合率。Objective: To investigate the effect of zoledronate acid treatment on bone healing of Bio-oss graft and osseointegration of dental implants in osteoporotic rats.Methods: Fourty female rats were randomly divided into three groups: sham-operation group(A,n=15),ovariectomization group(B,n=15) and zoledronate-treatment group(C,n=10).Animals in group B and C received bilateral ovariectomy,those in group A underwent sham-operation.Twelve weeks after surgery,femurs of rats from group A and B were examined for Bone Mineral Density(BMD) and histological changes to verify osteoporotic status.Then HA coated titanium implants were inserted into proximal metaphyses of tibiae and bone defects were made near the implants with simultaneous transplantation of Bio-oss particles.In group C zoledronate acid immobilized implants and Bio-oss particles were used.Animals were killed at 4 and 12 weeks after implantation surgery and undecalcified sections were prepared and examined histologically and histomorphometrically.Results: Osteoporotic status was verified in ovariectomized rats by significantly decreased BMD(P0.01) in femur compared with that in sham-operated animals.At both 4 and 12 weeks after implantation surgery,TCB,IBCR,NBV and ERBP in group B were all significantly decreased compared with those in group A and C(P0.01);while most of bone indices(IBCR,NBV,ERBP) in group C were significantly higher than those in group A(P0.01),except TCB,which was significantly decreased(P0.01).Conclusion: Experimental osteoporosis can impede bone healing of Bio-oss bone grafts and decrease implant-bone contact rate,treatment of bone grafts or dental implant with zoledronate acid may resist the negative influence of osteoporosis and promote bone graft healing and osseointagration of the implant.
