苦黄注射液对黄疸型病毒性肝炎退黄降酶效果及安全性的系统评价  

Jaundice-abating and Enzyme-lowering Effect and Safety of Kuhuang Injection for Icteric Viral Hepatitis:A Systematic Review

摘　　要：目的:系统评价苦黄注射液对黄疸型病毒性肝炎退黄降酶的有效性和安全性。方法:计算机检索Cochrane图书馆、PubMed、中国生物医学文献数据库(CBM)、维普中文科技期刊数据库(VIP)、中国期刊全文数据库(CNKI)、中文生物医学期刊文献数据库(CMCC)及万方数据库,对纳入的随机及半随机对照试验进行质量评价,并用RevMan5.0软件进行Meta分析。结果:共纳入7项研究,合计患者726例。对于急性黄疸型肝炎,苦黄注射液降低总胆红素(TBIL)和丙氨酸氨基转移酶(ALT)疗效与腺苷蛋氨酸相当[WMD=16.69,95%CI(-4.11,37.48)]和[WMD=14.87,95%CI(-90.49,120.22)];对慢性肝炎,其降TBIL和ALT疗效均不及腺苷蛋氨酸[WMD=41.87,95%C(I31.12,52.63)]和[WMD=79.64,95%CI(50.06,109.22)];亚组分析显示,剂量大于30mL组降ALT效果与腺苷蛋氨酸相当[WMD=58.49,95%C(I-34.45,151.43)]。现有资料未提示严重不良反应。结论:苦黄注射液治疗黄疸型病毒性肝炎有一定的退黄降酶效果,增加剂量可能提高降酶疗效。其疗效和安全性尚待更多高质量研究予以进一步证实。OBJECTIVE：To systematically evaluate the jaundice-abating and enzyme-lowering effect and safety of kuhuang injection for icteric viral hepatitis. METHODS：Retrieved from Cochrane library, PubMed, CBM, VIP, CNKI, CMCC and Wanfang database, included randomized controlled trails（RCT）and qusi-randomized controlled trails（qRCT）were evaluated and analyzed using RevMan 5.0 software. RESULTS： 7 studies were included, involving 726 patients. It showed that kuhuang injection provided similar efficacy to S-Adenosyl-L-Methionine （SAMe） in decreasing TBIL and ALT in patients with acute icteric viral hepatitis [WMD=16.69, 95%CI（-4.11, 37.48）] and [WMD=14.87, 95%CI（-90.49, 120.22）]; but inferior to SAMe in patients with chronic hepatitis [WMD=41.87, 95%CI（31.12,52.63）] and [WMD=79.64,95%CI（50.06,109.22）]. Subgroup analysis showed that Kuhuang injection over 30 mL was similar to SAMe for ALT reduction [WMD=58.49,95%CI（-34.45,151.43）]. No serious adverse drug reaction was reported. CONCLUSIONS：Kuhuang injection may abate jaundice and lower enzyme in patients with icteric viral hepatitis. The increase of dose may be better for decreasing ALT level. While more high-qualified studies are required to validate the therapeutic efficacy and safety.
