双侧上尿路移行细胞癌的诊治体会（附5例报告）

摘　　要：【目的】探讨双侧上尿路移行细胞癌患者的诊断与治疗方法。【方法】回顾性分析5例双侧上尿路移行细胞癌患者的临床资料。其中4例接受手术治疗，1例选择保守治疗。【结果】病理资料显示均为上尿路移行细胞癌。所有患者随访5～61个月。随访期间，未行上尿路手术的1例患者膀胱癌发生2次，其余患者中1例死亡，1例术后保留肾脏侧输尿管肿瘤复发，2例未发现肿瘤复发及转移迹象。【结论】为避免术后透析治疗，对于双侧上尿路移行细胞癌的患者应尽量选择保留肾脏手术。术后应进行严密的随访和观察。[Objective] To explore the diagnosis and treatment of transitional cell carcinoma of bilateral upper urinary tracts. [Methods]The clinical data of 5 patients wifh transitional cell carcinoma of bilateral upper urinary tracts were analyzed retrospectively. Among all patients, 4 cases underwent surgical treatment and one case received conservative treatment. [Results] The pathological results indicated that all patients were transitional cell carcinoma of upper urinary tract. All patients were followed for 5-61 months. During the follow up, one case receiving conservative treatment had the occurrence of bladder tumor twice. In other cases, one case died, and one case had the recurrence of ureter tumor in the side of reserved kidney, and two cases had no tumor recurrence and metastasis. [Conclusion] In order to avoid postoperative dialysis, the operation of kidney reservation should be selected for transitional cell carcinoma of bilateral upper urinary tracts. Intensive observation and follow up after operation should be taken.
