摘　　要：目的:探讨2型糖尿病(T2DM)合并非酒精性脂肪性肝病(NAFLD)患者血浆chemerin与炎症的关系。方法:采用酶联免疫法(ELISA)检测T2DM合并NAFLD患者(A组)、T2DM不合并NAFLD患者(B组)及正常对照组(C组)血浆chemerin水平,同时测定3组血糖、血脂、糖化血红蛋白(HbA1c)、空腹胰岛素(FINS)、内脏脂肪、高敏C反应蛋白(hs-CRP)、肿瘤坏死因子(TNF)-α、白细胞介素(IL)-6,计算稳态模型胰岛素抵抗指数(HOMA-IR),分析血浆chemerin与炎症指标的关系。结果:A组、B组、C组血浆chemerin水平分别为(130.87±33.84)μg/L、(115.97±21.82)μg/L、(87.01±9.63)μg/L,A组、B组高于C组,A组高于B组(均P<0.01)。A组患者血浆chemerin水平与腰围、低密度脂蛋白胆固醇(LDL-C)、内脏脂肪、TNF-α、IL-6呈正相关;多元逐步回归分析显示内脏脂肪、TNF-α、IL-6均为血浆chemerin水平的影响因素。结论:Chemerin可能在T2DM伴NAFLD的炎症反应中起重要作用。Objective：To investigate the relationship between plasma chemerin and inflammation in patients of type 2 diabetes mellitus（T2DM） with nonalcoholic fatty liver disease（NAFLD）.Methods：The plasma levels of chemerin were detected by enzyme-linked immunosorbent asssy（ELISA） in all subjects of group A（T2DM with NAFLD）,group B（T2DM without NAFLD） and group C（nomal group）.At the same time the levels of blood glucose,blood lipids,glycated hemoglobin（HbA1c）,fasting insulin（FINS）,visceral adipose tissue,high-sensitivity C-reactive protein（hs-CRP）,tumor necrosis factor（TNF）-α and interleukin 6（IL-6） were detected in three groups.The homeostasis model assessment for insulin resistance（HOMA-IR） was calculated and the relationship between plasma chemerin and inflammation was analyzed.Results：The plasma levels of chemerin were（130.87±33.84） μg/L,（115.97±21.82） μg/L and（87.01±9.63） μg/L in group A,group B and group C respectively.The plasma level of chemerin was significantly higher in group A and group B compared with that of control,and which was higher in group A than that of group B（P 0.01）.The plasma chemerin level in T2DM with NAFLD was positively correlated with waist circumference,low density lipoprotein cholesterol（LDL-C）,visceral adipose tissue,TNF-α and IL-6.Multiple stepwise regression analysis showed that visceral adipose tissue,TNF-α and IL-6 were independent related factors in influencing the level of plasma chemerin.Conclusion：Chemerin may play an important role in the inflammatory reaction of T2DM with NAFLD.
